Full Job Description

Compass Healthcare Consulting & Placement is conducting a search for an experienced MLTC Intake RN with UAS-NY experience for a Managed Care Plan servicing New York. Intake Assessments are Field & Remote Assessments. Qualified candidates will be able to visit members in their homes. Current Openings Available in the Bronx/Manhattan, Bronx/Westchester, Brooklyn/Staten Island or Queens/Nassau territories. Qualified candidates will have prior UAS RN experience, have a BSN and bilingual ability in Chinese, Spanish, Punjabi or Hindi is required for certain areas.

As an MLTC Intake RN, you will assess potential new members for eligibility and explain the program to them and their families. You will play a crucial role in ensuring a seamless transition to managed long-term care.
Key Responsibilities:

Conduct outreach activities within the community to assess the eligibility and safety of potential members.
Collaborate with the Intake/Outreach team and clinical team to ensure appropriate services are provided to new members.
Communicate effectively with potential members, families, referral sources, and physicians.

Qualifications:

Current New York State Registered Nurse License.
Bachelor’s degree in Nursing, BSN required.
One (1) year of experience in an acute care setting.
Must have UAS-NY RN experience.
Home care and management experience preferred.
Proficient in MS Office.
Strong oral, written, and interpersonal communication skills.
Bilingual Spanish, Bilingual Chinese, Bilingual Punjabi or Bilingual Hindi required
