Full Job Description

Job Overview

Seeking a PRN (as-needed) Physical Therapist to help manage case loads or fill in when needed. Possibility of available shifts across 3 different local clinics.

Duties

Conduct comprehensive patient assessments to determine functional limitations and develop individualized treatment plans.
Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics interventions, neurology strategies, and geriatrics care.
Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate patient care and discharge planning.
Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards.
Educate patients and their families on treatment goals, exercises, and home care strategies to promote ongoing recovery and community reintegration.
Manage discharge planning processes by evaluating patient progress and adjusting treatment plans accordingly for optimal outcomes.
Participate in outpatient clinic activities or inpatient hospital rounds as needed to ensure seamless continuity of care.

Qualifications

Valid state licensure as a licensed Physical Therapist with practicing in a physical therapy role.
Proven experience in outpatient clinics, inpatient hospital settings, or home health environments with a focus on patient rehabilitation.
Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards.
Experience working with diverse populations including geriatrics, pediatrics, orthopedics, neurology, and community care settings.
Familiarity with EMR (Electronic Medical Records) systems such as EMR or EHR management platforms.
Demonstrated ability to develop treatment plans that incorporate discharge planning and care coordination.
Excellent client communication skills to effectively educate patients on therapy exercises and health management strategies.
