As a national leader in helping young people and families, we also proactively identify unmet social services needs and develop the necessary partnerships to address those needs.

SUMMARY of POSITION:

Provide trauma informed coordination of services for Kinship Guardian Assistance Program (KinGAP) and state funded kinship subsidy families in the areas of assessment, family discovery and engagement activities, case management, supportive counseling, crisis intervention, advocacy, respite, peer support, education and services to support permanency.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Reviews referral information and completes comprehensive assessment of family and child strengths, needs, trauma and attachment issues, and current risk to family stability and permanency.
Verifies family’s eligibility for the program based on KinGAP and state funded kinship subsidy program enrollment.
Create family directed service plans, and complete assessments for kinship families and children to ensure alignment with family service plans and program goals.
Identifies psychoeducation needs of kinship caregivers related to trauma, attachment, parenting strategies, mental health, and available community resources, etc.
Provide strengths-based, trauma informed, family-centered, and culturally competent services.
Develop, deliver and/or identify community services, resources, and support needed by kinship families and guide families in navigating systems to access services.
Participate in and/or facilitate required meetings to support and advocate for family stability such as FAPT, treatment team meetings, IEP, court, etc.
Work with youth and families to create teams, develop goals, and monitor progress to ensure family stability and permanence.
Facilitate peer support groups, training and respite activities for kinship families.
Maintain caseloads and case file documentation based on established program protocol.
Completion of monthly, quarterly and annual reporting based on established program protocol
Maintain ongoing communication with the family and providers to ensure progress.
Develop and manage protocol for Family Fund in accordance with program guidelines and coordinate with ASM and Finance Office to secure payments on behalf of kinship families.
Participate in program, agency and regional committee meetings or meetings with community professionals as appropriate; conduct presentations at a local, state and national level as needed.
Cultivate and maintain collaborative relationships with external agencies and stakeholders to support program effectiveness and referral partnerships.
Share responsibility and respond to after-hours on call events and emergencies as assigned by program.
Ability to work a flexible schedule, including nights and weekends, to meet the needs of families.
Responsible for accomplishing 2-3 measurable annual Individual Key Performance Indicators (KPIs), providing concrete data to illustrate progress on 90-day Individual Priorities and participating in Team Huddles.
Other duties as assigned.

EDUCATION AND/OR EXPERIENCE:

Master’s degree (MSW) in social work with a minimum of one-year direct service experience or a Bachelor’s degree (BSW) in social work with two years direct service experience. Demonstrated ability to apply sound clinical casework and counseling skills to children and families, understand treatment modalities and their implications for services to families and youth. Completion of “High Fidelity Wraparound” training preferred.
