Job Overview

Join our dynamic primary care team as a full-time Chronic Care Management Nurse / Medical Assistant dedicated to delivering exceptional in-person patient care. This is an in clinic position, full time and you will be responsible for a complete caseload. In this vital role, you will collaborate closely with healthcare professionals in the clinic to manage complex chronic conditions, coordinate patient care plans, and ensure seamless clinical operations. Your expertise will help improve health outcomes, empower patients in their wellness journeys, and support our mission to provide compassionate, comprehensive healthcare. This position offers an energetic environment where proactive engagement and patient-centered service are at the forefront of everything we do.

Duties

Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to inform care plans.
Connect with patients monthly to update their Plan of Care, assist in medication refills, appointments and other requests
Manage patient intake processes efficiently, ensuring accurate documentation within electronic health records (EHR) systems while maintaining HIPAA compliance.
Assist with diagnostic evaluations and coordinate follow-up care, discharge planning, and post-acute care services.
Collaborate with healthcare professionals across disciplines—such as physicians, specialists, and behavioral health teams—to develop and implement personalized treatment strategies.
Support case management activities by monitoring patient progress, managing medical records, and facilitating communication between patients and providers.
Perform clinical procedures within scope of practice, including medication administration, wound care, and other routine interventions.
Engage in patient education efforts to promote health literacy, medication adherence, and lifestyle modifications for chronic disease management.

Experience

Experience with Medicare's Chronic Care Management programs provides a strong foundation for this role and is required.
Knowledge of health regulation policies including HIPAA standards and medical terminology is essential for ensuring compliance and accurate documentation.
Proven ability to perform patient assessments in inpatient or outpatient settings while collaborating seamlessly with multidisciplinary teams.
Familiarity with electronic health records (EHR) management systems is required to streamline clinical workflows such as ATHENA.
Strong background in managing patients with geriatrics, behavioral health issues, or chronic illnesses such as diabetes or cardiovascular conditions is advantageous.
