Ascendo Healthcare Staffing is actively seeking an experienced Home Health Registered Nurse, RN for an exciting 13-week contract opportunity near Terrebonne Parish, LA! This opportunity is moving quickly, and interviews are being scheduled immediately!
NOTE: This position is specifically for a skilled Home Health RN. Candidates whose experience is limited to Home Care, private duty, sitter care, continuous care, or non-skilled support work may not be the right fit. Qualified candidates must have experience with clinical assessment, case management, OASIS, and home health documentation. An ideal candidate should possess excellent skills in HCHB, OASIS, scheduling, and time management.
WHAT WE ARE LOOKING FOR

Required Qualifications:
- Active RN License Louisiana or Compact Nursing License
- Minimum 2+ years of recent Home Health experience (Geriatrics ONLY)
- Homecare Homebase (HCHB) experience
- OASIS documentation experience within the last 3 years
- Strong clinical assessment and critical thinking skills
- Excellent customer service and communication skills
- Reliable transportation and strong time management abilities

POSITION DETAILS
- Position: Home Health Registered Nurse (RN)
- Productivity Expectations: 30 Units Per Week | 6 Points Per Day
- Contract Length: 13 Weeks
- Start Date: 2 Weeks From Offer Date or ASAP
- On Call Required if Requested
Compensation:
- Earn up to $115,000 per year PLUS Mileage Reimbursement ($2200+/week)
- Weekly Pay: Via Direct Deposit Every Friday Morning
- Benefits: Medical, Dental, Vision & More (Single & Family Plans Available)
Schedule:
- Monday - Friday 8AM-5PM
- Productivity Expectations: 30 Units Per Week =40 HRS
- Note: Our client is looking for Home Health RN's that can maintain a full caseload of 30 productivity points per week. No more guessing! Full-Time Hours!
RESPONSIBILITIES
- Complete comprehensive patient assessments and develop individualized care plans
- Manage a home health caseload while maintaining quality outcomes and productivity standards
- Perform Start of Care (SOC), Resumption of Care (ROC), Recertifications, and OASIS documentation
- Coordinate care with physicians, therapists, caregivers, and interdisciplinary teams
- Provide patient and family education regarding disease management and treatment plans
- Monitor patient progress and communicate changes in condition appropriately
- Participate in on-call triage rotation as assigned
- Maintain accurate and timely electronic documentation utilizing Homecare Homebase (HCHB)
