JOB DESCRIPTION

POSITION: Residential Transitions Coordinator

DEPARTMENT: Residential Transition Services

EXEMPTION STATUS: Full-Time Exempt

HOURS OF WORK: 32 Hours

SUPERVISOR: Residential Transition Program Manager

The Coalition for Barrier Free Living (CBFL) is a nonprofit organization committed to advancing independent living, self‑advocacy, and equal access for people with disabilities and chronic conditions. Our work is rooted in the Independent Living (IL) philosophy and guided by consumer choice, peer support, and community inclusion—especially for individuals from historically marginalized communities.

About the Role

The Residential Coordinator works directly with adults with disabilities who are seeking stable, accessible housing and increased independence in the community. This position focuses on practical, hands‑on support—helping consumers navigate housing systems, complete required documentation, build service plans, and work toward goals they define for themselves.

This is a community‑based position that involves regular travel throughout Houston and surrounding areas to meet with consumers, housing providers, and partner organizations. You must have reliable transportation.

What You’ll Do

Apply and advocate for the Independent Living philosophy in daily work
Partner with consumers to develop service and housing plans based on their individual goals
Assist with locating and accessing income‑restricted housing, Houston Housing Authority programs, transitional housing, and managed care services
Provide case management, including assessments, documentation, service coordination, and follow‑up within required timeframes
Offer peer mentoring, referrals, and orientation to independent living and disability‑related resources
Make purchases and deliver items to support relocation
Meet with consumers to review progress, address barriers, and adjust plans as needed
Maintain accurate, timely, and confidential consumer service records in compliance with HIPAA and funding requirements
Track participation and submit required documentation and reports
Build and maintain relationships with community partners and service providers

What We’re Looking For

Required Qualifications

Bachelor’s degree in a human services field or four (4) years of related experience working with seniors or persons with disabilities
Two to four (2–4) years of experience providing services to individuals with disabilities
Ability to work independently and manage multiple consumer cases
Strong communication, organizational, and problem‑solving skills
Proficiency with MS Office, Excel, and agency‑supported software
Valid driver’s license, current insurance, reliable transportation, and ability to travel throughout the Houston area

Preferred Qualifications

Person with a disability
Experience working in the housing industry (not real estate)
Experience in Independent Living or related advocacy‑based settings
Experience developing or delivering trainings
Bilingual (Spanish preferred)
