JOB SUMMARY

Under the supervision of a higher classified employee, the Community Health Worker (CHW) will perform a variety of outreach and customer service activities in both an office and non-office setting. The CHW role exists to build strong, mutually-beneficial relationships with community members. These connections can increase access to available health services and can support changes to programs and services where needed to better support community health for all. Additionally, the CHW may provide support to all divisions within the Health Department, such as acting as a Health Department representative in a variety of community settings.

DEPARTMENT-WIDE CORE COMPETENCIES

Applies ethical principles in accessing, collecting, analyzing, using, maintaining, and disseminating data and information.

Contributes to implementation of organizational strategic plan.

Communicates in writing and orally with linguistic and cultural proficiency (e.g., using age-appropriate materials, incorporating images.

Describes the concept of diversity as it applies to individuals and populations (e.g., language, culture, values, socioeconomic status, geography, education, race, gender, age, ethnicity, sexual orientation, profession, religious affiliation, mental and physical abilities, historical experiences)

Collaborates with community partners to improve health in a community (e.g., participates in committees, shares data and information, connects people to resources.

Describes how public health sciences (e.g., biostatistics, epidemiology, environmental health sciences, health services administration, social and behavioral sciences, and public health informatics) are used in the delivery of the 10 Essential Public Health Services.

Adheres to organizational policies and procedures.

Incorporates ethical standards of practice (e.g., Public Health Code of Ethics) into all interactions with individuals, organizations, and communities.

EXAMPLES OF DUTIES

Essential Duties:

Provides client-centered support for Washtenaw County Health Department programs and services in a community, home or clinical setting. Some examples may include: community lead testing, direct observed therapy, program enrollment, translation or other support as needed.

Supports case management goals by working to include client voice and preferences in planning.

Participates in the planning and implementation of client care plans.

May complete home or community visits, alone or with other staff, to provide services or engage with community members.

May teach skills needed to increase independence in the areas of work skills, safety skills, social skills, finding and maintaining housing, and receiving and maintaining benefits.

Works to secure needed services for the clients from any health, mental health, or social service provider. This may include advocating for systems changes that better support clients' health or needs.

Provides community education and outreach for health department services.

Develops relationships with community partners to support health and the values outlined in the department's strategic plan.

Provides clerical support as needed.

Provides services in a culturally competent manner. Examples may include: providing written and spoken materials in clients preferred language and utilization of interpretation services.

Participates in individual, program and department processes with the goal of continuous quality improvement.

Provides timely and accurate documentation for services provided.

Updates and maintains a list of community services and agencies for referral purposes.

Performs related duties as assigned.

EMPLOYMENT QUALIFICATIONS

Knowledge, Skills and Abilities:

Ability to understand and apply an array of department and county policies, procedures, rules and regulations.

Ability to organize, prioritize and coordinate work with minimal supervision.

Ability to document interactions using written communication with clear English and proper grammar, spelling and punctuation.

Ability to use a wide variety of computer software including, but not limited to, Microsoft Office products, Outlook and social media such as Facebook and Twitter. May receive additional training on state and department required systems.

Ability to work cooperatively in a team and/or community setting while establishing and maintaining effective working relationships.

Ability to provide quality client centered services under changing and challenging circumstances in an accepting and non-judgmental way.

Ability to maintain client confidentiality.

DESIRED QUALIFICATIONS

Proficiency in a second language.

Community Health Worker Certification recognized by the State of Michigan.

Strong community connections and/or similar life experiences to the community or population served.

Experience working with people.

LICENSES AND CERTIFICATIONS

Possession of a valid Michigan vehicle operator's license. Availability of automobile, van, or truck.

PHYSICAL DEMANDS

Duties require sufficient mobility to work in a both a normal office setting and in the community. Use of standard office equipment including as well as specialized devices to communicate with hearing and speech impaired clients. Requires ability to read printed material and must be hearing and speech sufficient to communicate in person or over the phone. Requires ability to drive to multiple work locations.

EDUCATION/EXPERIENCE

Possession of a high school diploma or equivalent. This is an entry-level classification; no specific prior experience is required.
