Location: Greater Twin Cities Metro Area | 21-County Service Area

Schedule: Per Diem or Full-Time | Weekend Rotation

Compensation: $19-$25/hour DOE + Mileage Reimbursement + $500 Sign-On Bonus

Hospice care is incredibly personal.

You'll often see the same patients consistently, giving you the opportunity to learn their routines, preferences, personalities, and what makes them comfortable.

That consistency matters.

It also means you may notice things others don't.

A patient seems weaker today.

They're eating less.

Their breathing looks different.

Something about their skin has changed.

They're communicating differently than they normally do.

Your observations can become important pieces of the clinical picture, and you'll work closely with the nursing team to make sure changes are communicated.

What You'll Do

Assist patients with bathing, grooming, dressing, hygiene, mobility, and activities of daily living.
Provide comfort-focused personal care based on individualized plans of care.
Observe and report changes in patient condition to the clinical team.
Provide companionship and reassurance to patients.
Help families feel supported and confident in the care their loved one is receiving.
Communicate closely with hospice nurses and other interdisciplinary team members.
Maintain a clean, safe, and comfortable patient environment.
Travel to patients throughout the Greater Twin Cities Metro Area.

Active Minnesota Certified Nursing Assistant (CNA) certification.

Strong observation and communication skills.

Valid driver's license and reliable transportation.
Willingness to travel throughout the Greater Twin Cities Metro Area.
