We are a growing Rural Health Care medical office in Valley Springs and we're growing faster than ever. We are looking to add a Care Management Coordinator Nurse to our fun and highly energetic team. Job Purpose:

The Care Management Coordinator at Mark Twain Health Care District is responsible for facilitating and coordinating comprehensive care management services for patients. This role involves working closely with healthcare providers, patients, and their families to ensure effective care plans are developed and executed to enhance patient outcomes and satisfaction.

Key Responsibilities:

Develop and implement individualized care plans in collaboration with healthcare providers and patients.
Coordinate and facilitate communication between patients, families, and healthcare teams to ensure seamless care delivery.
Monitor patient progress and adjust care plans as necessary to meet changing needs and conditions.
Educate patients and families about care plans, available resources, and self-management strategies.
Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations and standards.
Identify and address barriers to care, including social, financial, and logistical challenges.
Collaborate with community resources and services to provide comprehensive support to patients.
Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improvement.
Ensure adherence to all organizational policies, procedures, and quality standards in care management activities.

Required Education:

Bachelor’s degree in Nursing or a related healthcare field.

Required Experience:

Minimum of 3 years of experience in a healthcare setting, preferably in care management or coordination.
Experience working with diverse patient populations and managing complex care needs.
Previous experience in a hospital or community health setting is highly desirable.

Required Skills and Abilities:

Strong understanding of care management principles and healthcare systems.
Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
Ability to work collaboratively in a multidisciplinary team environment.
Proficient in using electronic health records and care management software.
Strong organizational skills with the ability to manage multiple priorities and tasks efficiently.
Demonstrated problem-solving skills and the ability to make informed decisions in a fast-paced environment.
Compassionate and patient-centered approach to care, with a commitment to improving patient outcomes.
