Full Job Description

JOB TITLE: Medical Case Manager

REPORTS TO: Regional Client Services Manager

STATUS: Regular Full-Time, Non-Exempt

COMPENSATION: $23/hour

Note: Compensation based on factors such as job duties, education level, position autonomy and supervisory responsibilities

POSITION SUMMARY:

The Medical Case manager will work closely with clinic staff to provide medical case management services in order to help clients overcome barriers to accessing medical care and creating/maintaining a healthy lifestyle.

Work involves providing supportive services to promote participant access to medical care and increase participant self-sufficiency; performing administrative tasks involved in the review and maintenance of a caseload of program participants. The principal duties are performed in an office environment where participants receive medical care.

RESPONSIBILITIES AND DUTIES:

This list is not exhaustive and may be supplemented as necessary.

Complete medical intakes for clinic patients, including needs assessments, patient mental health questionnaire and appropriate referrals for supportive assistance.
Determine eligibility for and enroll participants in Ryan White Part B program according to standards set forth by the Ryan White Part B Medical Case Management manual.
Maintain complete and accurate records for all participants according to standards set forth by the Ryan White Part B Medical Case Management manual.
Provide supportive services as appropriate according to the Ryan White Medical Case Management Standards of Care.
Support enrollment and management of patient assistance programs as needed including the Insurance Assistance Program and navigation to insurance enrollment support.
Assess barriers to accessing medical care and maintaining medication adherence.
Develop a comprehensive case management plan with clients. This shall include intake interview to determine client’s needs, goals, and eligibility.
Provide mediation and advocacy with service providers on the client’s behalf to develop a workable plan to maintain access to medical care.
Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. medical provider, medical office staff, peer navigator, referral sources, collaborating agencies).
Provide information and referral assistance regarding available support from appropriate social service agencies and/or community programs.
Identify participant strengths and barriers to stability and assist participants to reducing barriers and linking to resources and services.
Provide pro-active follow-up after medical visits to ensure client satisfaction and understanding of follow-up medical care plan, referrals and physician instructions.
Maintain accurate daily logs records, monthly outcome reports, and files for each client.
Arrange transportation for clients to medical appointments as deemed necessary.
Complete all appropriate data reporting forms.
Maintain a complete working file providing activity documentation and copies of all corresponding paperwork.
Support enrollment and management of patient assistance programs as needed
Assume other responsibilities as assigned.

JOB QUALIFICATIONS AND SKILLS:

· Excellent communication skills, particularly listening, mediation, and writing skills.

· Possess strong organizational skills with ability to meet a demanding workload.

· Detail oriented to complete requirements of files and contract compliance.

· Creative thinker/adaptive personality.

· Concentrated record keeping, budgeting, and mathematical skills; ability to produce required reports to federal, state. and local government agencies and funding sources.

· Experience with computer and knowledge of Microsoft Office.

· Sensitivity to cultural and socioeconomic characteristics of population served.

· The ability to put your own personal judgments, prejudice, and biases aside and work with clients with diverse life experiences.

· A conviction about the capacity of people to grow and change.

· The ability to establish and set appropriate limits with persons served to help them gain skills and confidence.

· The ability to work collaboratively with other personnel and/or service providers or professionals.

· The capacity to maintain a role to empower clients and to intervene appropriately to meet service goals.

· May be required at times to work a hybrid work schedule and must have the ability to do some work from home.

EDUCATION AND EXPERIENCE:

People with lived experience of substance use, incarceration, homelessness, and sex work; people of color; members of the LGBTQIA+ community; and people living with HIV and/or hepatitis C are encouraged to apply.
Requires knowledge of Ryan White Program and belief in Ryan White program philosophy and strategies.
Minimum Education Requirements: bachelor’s degree in a human services field (social work, psychology, sociology, etc).
