General Functions: Provide direct support services to adult males living in a mental health 24-hour supervised residential program. Oversee the daily operations of assigned group home, provide supervision to assigned workers, and support residents to achieve their goals as they strive for greater independence in the community.

Specific Functions:

Assist the Case Manager in monitoring and implementing Individual Treatment, and Recovery Plans to ensure client progress in meeting individual goals.

Participate as a member of the interdisciplinary team in the process of planning clients' Individual Treatment and Recovery Plans.

Deliver and document home, facility, and community-based mental health services as indicated by the Individual Treatment and Recovery Plans of persons served with mental health issues. Services may include Community Support, Crisis Intervention, Case Management, Medication Training, etc.

Meet or exceed MRO billing quota as determined by Director of Behavioral Health.

Provide staff training and re-training, as needed, to assist Direct Support Professionals to complete daily MRO biilling in order for them to meet or exceed MRO weekly billing quotas.

Complete required documentation correctly and timely, including psychiatric review notes, daily MRO notes for each shift, case notes, task analysis notes, room inspections, safety inspections, etc.

Supervise and assure completion of individual daily activity schedules as indicated by the Individual Treatment and Recovery Plans, including daily grooming and hygiene, meal planning, and meal preparation, etc.

Provide skill training for clients as indicated by the Individual Treatment and Recovery Plans, including cooking, meal planning, housekeeping, kitchen chores, laundry, personal shopping, banking, etc.

Oversee client finances, teaching clients budgeting and banking skills, and insuring that monthly budgets are completed timely.

Assist in coordination of care related to the clients' physical health. Schedule medical appointments as required or as needed, and serve as client advocate during appointments, maintaining records and completing all required paperwork.

Monitor and document clients self-administration of prescribed medications and treatments.

Assist in the development of activity schedules along with clients, consistent with Individual Treatment and Recovery Plans.

Participate in monthly clinical supervision meetings, house meetings, and psychiatric consultation meetings.

Attend Behavioral Health meetings and trainings as directed by the Case Manager or Coordinator.

Develop a solid working knowledge of client benefits, including social security, SNAP, Medicaid, Medicare, Joliet Housing Authority, etc.

Assist the Case Manager with managing client benefits, including accompanying clients on appointments, completing applications and redetermination forms, and communicating with staff of other agencies as directed by the Case Manager.

Assist the Case Manager and other residential supervisors in the overall operation of the clients' home within the established policies and procedures.

Function as the liaison to the Case Manager regarding building maintenance issues and the need for consultants to provide services to the clients of the home.

Supervise, train, evaluate, and discipline staff, providing for staff development as indicated.

Coordinate and develop the monthly staff schedule and arrange for replacement workers in the event of planned or unplanned staff absences and/or vacancies.

Provide coverage for staff absences and/or vacancies by adjusting own work hours, as needed.

Be available on rotating weekends to respond to staff absences and arrange for replacement workers in the event of planned or unplanned staff absenced and/or vacancies.

Communicate appropriately and effectively with consultants and other agency staff. Respond appropriately to clients' family members, friends, and guests.

Provide counseling and crisis intervention to clients in need of such assistance and guidance.

Assist in the implementation of client meetings in accordance with the current policies and procedures, including safety meetings and Resident Council meetings.

Assist in the implementation of fire drills, safety checklists, disaster drills, and other safety procedures.

Maintain ongoing compliance with all agency-related training and certification requirements.
Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
Operate agency or personal vehicle in accordance with established policies and procedures.
Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures.
Other duties as assigned.

Qualifications: Mental Health Professional preferred. Prior supervisory experience desired. Must be 21 years old or older and have a high school diploma or equivalent with a minimum 1 year of experience in human services, with demonstrated leadership ability. Driving record and auto insurance in accordance with agency policy. A post offer physical and criminal background check required. Reliable transportation required. EOE.
