General Functions:

Registered Nurse provide services, consultation, and guidance to staff and participants with regard to behavioral health programs.

Specific Functions:

Complete nursing assessments, including review of medical, clinical charts and/or other pertinent documents as well as interviews with the individuals, family members, guardians, and/or significant others, as appropriate, about past and present functional levels, capabilities and performances, including prior community-based experiences and circumstances of admissions to SMHRF/NF. Complete narrative reports of these assessment outcomes with recommendations.

Offer consultation and guidance to staff with regard to physical and mental health concerns of individuals served.

Assist in identifying and setting up appointments with specialists, as needed.

Ensure continuity of healthcare/services and medical appointments.

Facilitate transfer of medications and linkages with primary healthcare, medical specialists, pharmacies, managed care organizations, and ancillary services/supports, as appropriate.

Develop, review, and authorize Comprehensive Service Plans based on the assessment and coordinate care and services for individuals transitioning to the community.

Provide transition preparation services to individuals still residing in SMHRF/NF as identified in the Comprehensive Service Plan.

Assist PMHNP with nursing assessments and coordination of medical needs of clients transitioning into the community.

Arrange for durable medical equipment and Home and Community-Based waiver services with Medicaid Managed Care health plan support, as appropriate.

Follow up within 7 days of an unscheduled ER visit or hospitalization and document.

Offer consultation in the development of program policies and procedures regarding health maintenance issues.

Document consultations and services with regard to both individual participants and the overall program.

Provide additional support, education, and training to individuals with extra support needs including, but not limited to, group home participants.

Monitor the distribution of medications and follow-up on medical problems/hospitalizations.

Advise and support the team about clinical and/or medical issues as they arise.

Assist in the assignment and oversight of cases referred to the program.

Participate as needed in Psychiatric Review, Clinical Consults, and Supervision meetings.

Administer prescribed injections, as needed.

Meet or exceed MRO billing quota as set by the department director.

Provide coverage assistance, as assigned, in the event of nursing absences/vacancies in the behavioral health department to ensure appropriate client care.

Provide mental health services including:

--Individual and Group Counseling

--Crisis Intervention

--Case Management

--Client-Centered Consultation

--Psychotropic Medication Administration

--Psychotropic Medication Training

--Community Support Individual

Travel to various program locations, as assigned.

Maintain ongoing compliance with all agency-related training and certification requirements.
Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
Operate agency or personal vehicle in accordance with established policies and procedures.
Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures.
Other duties as assigned.

Qualifications: Licensure as a Registered Nurse with one year of experience and knowledge of pharmacology. Psychiatric experience required. Driving record and auto insurance in compliance with agency policy. A post-offer physical and criminal background check required. Reliable transportation required. EOE.

Pay: $35.90 per hour
