Overview
The Registered Dietitian/Diabetes Educator at Riverside Healthcare plays a critical role in providing expert diabetes education and nutritional counseling to patients. This position serves as a key resource for both the facility and the community regarding diabetes and nutrition information. The Registered Dietitian/Diabetes Educator assesses patients nutritional needs, educates them on managing diabetes through diet, and collaborates with physicians and other healthcare professionals to ensure optimal care. Additionally, the role involves leadership in advancing diabetes care practices and providing community outreach through educational programs. Demonstrates flexibility with assignments within professional scope/duties/licensure.

Essential Duties
Patient Assessment & Education:
- Assess patients' nutritional needs, including conducting blood pressure checks, foot exams, and waist circumference measurements.
- Interview, evaluate, and educate individuals with diabetes on appropriate dietary modifications and diabetes management techniques.
- Reassess patients' learning needs through active case management to ensure ongoing education and support.
Collaboration & Consultation:
- Collaborate with physicians and other healthcare professionals to develop and implement individualized nutritional and medical management plans for diabetic patients.
- Act as a diabetes nutrition expert by providing leadership and guidance to multidisciplinary healthcare teams to facilitate positive changes in patient care.
Community & Organizational Outreach:
- Conduct in-services and educational programs for healthcare staff and the community to share the latest diabetes treatments and nutrition practices.
- Serve as a resource for both the facility and the broader community on diabetes and nutrition-related topics.
Documentation & Reporting:
- Provide accurate, timely documentation of patient interactions and services in medical records.
- Prepare and submit reports to physicians on patient progress and management outcomes.
- Provide monthly reports on patient education and outcomes to the Director as assigned.
Program Development & Leadership:
- Share responsibility in planning, implementing, and controlling work processes to ensure the diabetes education program meets quality standards.
- Stay updated on education requirements for maintaining a high-quality diabetes education program, ensuring compliance with industry standards.
Non-essential Duties
- Assist in special projects or assignments as required by the department.
- Participate in continuing education programs and attend professional development seminars to maintain expertise in diabetes care and nutrition.
- Provide support for other dietary or diabetes-related services as needed.

Preferred Experience
- Experience with patient counseling, diabetes management, and individualized meal planning.
- Knowledge of current diabetes treatments, medical nutrition therapy, and best practices in diabetes care.
- Strong communication and leadership skills with the ability to collaborate effectively with other healthcare professionals.
- Ability to provide compassionate, patient-centered care while maintaining a professional and organized work environment.
Required Licensure/Education
- Bachelors degree in Nutrition, Dietetics, or a related field is required.
- Masters degree in Nutrition or Public Health.
- Certified Diabetes Care and Education Specialist (CDCES) is required or must be obtained within 3 years of employment.
- Commission On Dietetic Registration
- A Licensed Dietitian (LD) in the State of Illinois is required.
Pay Range
USD $31.40 - USD $41.21 /Hr
