Job Overview

The Case Coordinator plays a vital role in supporting youth at the Juvenile Stabilization Center (JSC) by managing case flow, coordinating services, and facilitating communication between internal teams, families, and external stakeholders. This position ensures that each youth receives individualized, trauma-informed, and timely care from intake to discharge. The Case Coordinator provides advocacy, service planning, and coordination across systems, helping to stabilize youth in crisis and promote successful reintegration or transition to long-term services.

Responsibilities

Coordinate the intake, ongoing care, and discharge planning for youth admitted to the JSC.
Develop, update, and maintain individualized service plans in collaboration with the clinical team and youth support staff.
Serve as the primary point of contact for families, referral sources, and partner agencies regarding case progress and youth needs.
Maintain accurate, up-to-date case documentation, including service notes, reports, and contact logs, in compliance with agency and licensing standards.
Participate in multidisciplinary team meetings, treatment planning meetings, and case staffings.
Attend court hearings, school meetings, and external case conferences as needed to support advocacy and case coordination.
Monitor each youth’s progress, identify service gaps, and initiate referrals to community-based resources and supports.
Facilitate communication and coordination between residential, clinical, educational, and medical staff.
Track and report outcomes related to youth stabilization, discharge status, and follow-up services.
Build and maintain relationships with external providers, community agencies, and family systems.
Participate in ongoing training and quality improvement efforts.
Perform other duties as assigned to support the effective operation and mission of the JSC.

Minimum Qualifications

Bachelor’s degree in social work, psychology, criminal justice, human services, or a related field.
At least two (2) years of experience in case management, youth services, child welfare, or behavioral health.
Familiarity with trauma-informed care and working with high-needs or system-involved youth.
Strong organizational and documentation skills.
Ability to work collaboratively with diverse teams and community systems.
Valid driver’s license and reliable transportation for local travel as needed.
Proficient in Microsoft Office and electronic record systems.

Preferred Qualifications

Master’s degree in social work, counseling, public administration, or a related field.
Experience working in a residential, crisis, or juvenile facility.
Bilingual abilities (English/Spanish or other languages spoken by youth served).
Experience with court systems, IEP processes, or coordinated service planning.
Familiarity with Kansas child welfare or behavioral health systems and regulations.
Experience with trauma-informed case management or care coordination frameworks.
