CHRISTUS Good Shepherd Medical Center- Longview and Lifepoint Rehabilitation, an operating division of Lifepoint Health, have formed a joint venture to operate Longview Rehabilitation Hospital.
With 36 private rooms, CHRISTUS Health Rehabilitation Hospital increases capacity for treating rehabilitation patients beyond the rehabilitation unit at CHRISTUS Good Shepherd Medical Center - Longview, allowing the current inpatient rehabilitation unit to be repurposed to support acute care demand.

How you’ll contribute:
The Licensed Case Manager is responsible for coordinating the interdisciplinary care plan, discharge planning, and patient progress communication within the inpatient rehabilitation setting. This role serves as a liaison among patients, caregivers, and the healthcare team to ensure timely and effective care transitions.
Essential Functions

Complete departmental orientation, as well as initial and annual competencies.

Collaborate with interdisciplinary team members to identify barriers to care or discharge and develop appropriate solutions.

Document patient care activities in alignment with workflow timelines, including completion of the Individual Plan of Care (IPoC) per CMS guidelines.

Schedule family conferences and communicate progress and discharge planning with caregivers after team conferences and as needed.

Coordinate weekly interdisciplinary team conferences to monitor treatment goals and outcomes.

Review assigned Case Mix Group (CMG) and assist the team in identifying actively treated comorbid conditions; communicate findings to the Health Information Management (HIM) team.

Perform other duties as assigned.

Ability to manage complex cases and work collaboratively with medical and non-medical staff.
Qualifications and requirements:
Education & License: Licensure required as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
Certifications: Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
Experience: Minimum of 2 years of experience in social work or case management in an inpatient setting preferred; acute rehabilitation hospital experience strongly preferred.
Skills: Effective oral and written communication in English; additional languages preferred. Proficiency in Microsoft Office applications required.

STRHS-Lawrenceburg, formerly Crockett Hospital, is a 99-bed, acute care facility located in Lawrenceburg, Tennessee, 85 miles south of Nashville.
We have been providing the best healthcare in Lawrence County and surrounding counties in Southern Middle Tennessee since 1974. We expanded the facility and services in 1991 with the addition of an Outpatient Surgery Unit, a state-of-the-art 24-hour physician-staffed Emergency Department, and an Intensive Care Unit. The hospital opened a 10-bed Physical Rehabilitation Unit in 1997, and a Women's Health Center in 1999. Diagnostic imaging services include in-house MRI, Nuclear Medicine, CT, X-ray, Digital Mammography and Ultrasound. The Rehabilitation Unit provides Physical, Occupational and Speech Therapies. In 2009, a $5.6 million construction was completed which expanded the Emergency Department, Outpatient Surgery Unit and Registration area.
