Full Job Description

Job Title: Lead Case Manager

Reports To: Clinical Director

Position Overview

The Lead Case Manager oversees and guides a team of case managers, ensuring patients receive compassionate and high-quality care. In addition to providing leadership, the Lead Case Manager also takes on the duties of a Case Manager, working directly with clients to assess their needs and coordinate essential services like counseling, nutrition, rehabilitation, transportation, and education. A primary focus is ensuring that care is comprehensive and cost-effective, benefiting the client and their insurance providers. The Lead Case Manager also provides continuity of care, acting as a strong advocate for each individual. In this role, you’ll regularly monitor and evaluate the performance of case managers, offering feedback and coaching to help them grow and work more efficiently.

Job Functions

● Monitor client’s progress through treatment

● Begin working with client upon admission and maintain support through discharge

● Provide support for client through primary therapist direction and treatment planning

● Determine any needs the client may be having while in treatment and report any pertinent clinical information to the treatment team.

● Participate in treatment team meetings to best assure continuity of care between clinical staff and the needs of the client.

● Assist clients with any specialized needs they may have while in treatment i.e. paying bills, family coordination, schooling, finding sober living and IOP programs, and any other direction given from primary therapist.

● Participate in weekly clinical team meetings to gain insight and oversight from the clinical director.

● Complete treatment plans, biopsychosocial assessments, and pre-certs under the supervision of a licensed counselor.

● Performs Administrator On Call (AOC) Duties and on call for urgent clinical needs of clients.

● Performs self-audit of charts as directed by Clinical Director

● Work with the Clinical Director to train and onboard new hires.

● Oversee the case management team to ensure that services are properly arranged, coordinated, and delivered to meet the needs of all clients.

● Assign and divide caseloads based on both the Case Managers' and clients' schedules.

● Review and monitor all Case Management documentation for each client from admission to discharge.

● Provide Case Managers with the necessary support, including resources, curriculum, and contacts to ensure they can effectively perform their duties.

● Performs other duties as assigned by Clinical Director.

Requirements

● Valid Driver’s License with clean driving record, as determined by Virtue’s insurance carrier

● Must hold at minimum a high school diploma

● Minimum 5 years of behavioral health and or case management experience required

● Must hold a minimum of 18 hours of behavioral health-related education

● Must be 21 years of age

● Knowledge of Windows, basic email, Microsoft Word, timekeeping software, and basic computer navigational skills.

● Ability to write professional correspondence and reports free from grammatical and spelling mistakes.

● Ability to read, analyze, and interpret emails, letters, policies, procedures, etc.

● Shift Flexibility

● Self-motivated and self-directed

● Team player

● Professional and/or personal experience with substance abuse treatment highly preferred

● High level of patience

● Active CPR and First Aid certification required

Must have reasonable ability to do the following:

· Stand, walk and sit during assigned shifts hours.

· Have the ability to manually handle patient belongings with gloved hands.

· Be able to manually assess a patient as part of a physical exam.

· Reach, climb or balance, stoop, kneel, crouch or crawl.

· Speak and have access to the special senses within normal limits (hear, taste, smell, etc.) as it applies to patient care.

· Must be capable of lifting up to 20 pounds.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
