Job Summary

We are seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) to serve as an MDS Coordinator, specializing in the management and coordination of the Minimum Data Set (MDS) assessments for long-term care and skilled nursing facilities. In this vital role, you will ensure accurate, timely completion of comprehensive resident assessments in accordance with federal and state regulations, including CMS guidelines. Your expertise will support optimal patient care, facilitate compliance with healthcare standards, and enhance clinical documentation processes. This position offers an exciting opportunity to leverage your nursing experience to improve resident outcomes and streamline documentation workflows within a dynamic healthcare environment. LPN may be required to work as charge nurse role as needed along with MDS role.

Responsibilities

Lead the MDS assessment process by coordinating comprehensive data collection, review, and submission in compliance with CMS regulations and long-term care standards.
Collaborate closely with interdisciplinary teams—including nursing staff, case managers, social workers, and therapy providers—to gather accurate clinical information necessary for MDS completion.
Ensure timely submission of all MDS assessments, including quarterly reviews, annual assessments, and significant change assessments, adhering to strict deadlines.
Review medical records, clinical documentation, and coding details such as ICD-10/ICD-9 codes, CPT codes, DRGs (Diagnosis-Related Groups), and other relevant data to ensure accuracy and completeness.
Maintain proficiency in EMR/EHR systems such as Wellsky and other electronic health record platforms for efficient documentation management.
Conduct documentation review to verify compliance with HIPAA regulations, Medicare requirements, NCQA standards, and state healthcare policies.
Stay current on healthcare regulations including CMS guidelines for Medicare and Medicaid services, long-term care regulations, hospital standards—including ICU and trauma center protocols—and health information management best practices.
Provide education and support to staff regarding clinical documentation standards and coding practices to improve accuracy and reimbursement processes.
Participate in audits and quality assurance activities related to medical records management and clinical documentation improvement initiatives.

Skills

Knowledge of EMR/EHR systems such as Wellsky and proficiency in Microsoft Office suite is preferred.
Strong understanding of medical terminology, physiology, anatomy knowledge, and clinical documentation standards necessary for accurate assessment completion.
Experience with managed care processes including utilization management and utilization review procedures.
Familiarity with CPT coding, ICD coding (ICD-10/ICD-9), DRG assignment, Medicare regulations, CMS policies, NCQA standards, long-term care regulations, HIPAA compliance requirements—and related health regulation policies.
Proven ability to review complex medical records accurately while maintaining attention to detail under tight deadlines.
Strong case management skills with a focus on discharge planning and patient care coordination across various healthcare environments including hospice care or emergency medicine settings.
Excellent communication skills to collaborate effectively with multidisciplinary teams while ensuring adherence to clinical documentation standards.
