Full Job Description

Team Select Homecare is looking for Licensed Practical Nurses to join our Team Select

family. Our nurses will document and provide skilled nursing care in a 1:1 setting

to our patients in their home.

Locations:

Lafayette

We are seeking a compassionate and skilled nurse to provide one-on-one care for a medically complex child in the home setting. The ideal candidate will have experience with tracheostomy care, ventilator management, and G-tube feeding, or be willing to receive specialized training. Responsibilities include monitoring the child's health status, administering prescribed treatments, supporting daily activities, and collaborating with the family and healthcare team to ensure safe, high-quality care.

This position offers the opportunity to build a meaningful relationship with a child and family while making a positive impact on the child's quality of life. Strong clinical skills, reliability, and a caring, patient-centered approach are essential.

Qualifications:

Active LPN license
Experience with trach, ventilator, and G-tube care preferred
Current CPR certification
Excellent communication and assessment skills
Compassionate, dependable, and family-focused

Responsibilities

Implement the nursing care plan for each patient.
Initiate preventive and rehabilitative nursing procedures.
Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
