$500 SIGN-ON BONUS - Half upfront, half after 6 months

GENERAL SUMMARY OF DUTIES:

DiaSante Health, LLC offers virtual patient care services in accordance with Medicare’s Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. RPM and CCM are comprehensive, patient-centered, and team-based approaches to managing chronic conditions and improving health outcomes, using a cellular-enabled patient monitoring device. Partnering with physician practices, DiaSante delivers CCM and RPM services to enrolled Medicare patients.

The Certified Medical Assistant (CMA) is responsible for initiating post-enrollment activities, ensuring patients receive their equipment, are properly set up, and understand how to use the devices, and facilitating initial and routine readings and nursing notes. The CMA is also responsible for providing RPM and CCM services to patients following established standards and practices.

A successful CMA will demonstrate excellent verbal communication skills and the ability to work across multiple platforms simultaneously. They will display professional behaviors, communicate effectively to others, and function as a member of the healthcare team under the general supervision of the physician.

JOB DETAILS:

Clinical Operations: Participate in the care of the patient, under the general supervision of the physician and in collaboration with the healthcare team

Collect patient information and assessment data
Ensure that patient chart entries are made accurately and in a timely manner; forwards charts as appropriate
Provide ongoing monitoring and follow-up of patients' progress and adherence to care plans
Confirm and clarify written orders prior to implementation of delegated tasks
Recognizes emergency situations and implements emergency procedures according to clinic guidelines
Provides appropriate patient specific education, and health information materials

Support Operations: In close partnership with the healthcare team, assure smooth and efficient patient flow and clinical operation

Make, receive, and handle medically related phone calls as directed by physicians
Notify patients of test results; schedule follow-up appointments, as needed
Conduct Remote Patient Monitoring (RPM)'s for physicians
Perform other tasks, as assigned

SKILLS:

Clinical experience with a solid grasp of medical terminology
Proficient in calmly and methodically explaining set-up procedures
Skilled in troubleshooting device-related issues
Quick learner with computer systems
Self-motivated with strong task prioritization and time management skills
Maintains a HIPAA-compliant workspace

Language Skills

Communicate effectively with others on the healthcare team
Understand medical terminology, anatomy and physiology, diagnostic procedures, pharmacology, and treatment assessments to the extent required to understand and accurately transcribe dictated reports

Computer skills

Ability to learn and use all functions of electronic medical record software
Must accurately enter data into a database, search for information, and send and receive email and attachments

EDUCATION REQUIREMENTS:

Nationally recognized certification as Medical Assistant
Two years of medical assistant experience may substitute for certification.

EXPERIENCE:

Previous medical office experience, preferred
EMR experience, preferred
Personal experience in care management

ENVIRONMENTAL/WORKING CONDITIONS:

In office/clinic primary with possibility of some occasional remote telework
Frequent telephone calls and other interruptions, which requires being able to handle a fast-paced work environment during time at work
Must be able to prioritize multiple projects
Must maintain strict confidentiality and exercise professionalism when interacting with staff, managers, supervisors, physicians, external customers, etc.

PHYSICAL/MENTAL DEMANDS:

Requires continuous standing, sitting, with frequent walking, bending, and stooping
May require lifting 10-20 pounds
Possible exposure to infectious diseases when in clinic setting

Pay: From $15.01 per hour
