Full Job Description

Pay is commensurate with experience within a range of $20.00 to $24.00 per hour. Full time, non-exempt, benefits eligible. Our mission at the Bert Nash Center as the Community Mental Health Center (CMHC) of Douglas County is to advance the health of the community through comprehensive behavioral health services responsive to evolving needs and changing environments. Responsibilities: The primary responsibility of SED Waiver Facilitator will be to organize and manage the components of the Waiver Program for children that meet the criteria for the targeted population. Specific duties include but not limited to:

acting as primary Waiver contact for the family and other professionals working with the client via the client's Plan of Care/POC (treatment plan);
monitoring the tasks outlined on the POC with other treatment team members, medication services staff, school and other community providers by communicating with such staff and community providers on a regular basis and coordinating schedules and assignments;
coordinating and scheduling respite care in accordance with client's needs;
acting as Treatment Plan Coordinator for assigned clients;
providing case management services, resource acquisition, crisis intervention, outreach activities and coordinate these services with other community health and social service agencies;
delivering services in a variety of locations, including but not limited to the client's home, the school, the Child & Family Services program site, or another community setting, on an as-needed basis;
providing transportation to and from various activities or appointments as indicated by the treatment plan or as instructed by the QMHP;
responding to crises while ?on call? in rotation with other similarly trained staff;
regular and predictable attendance is an essential function of the job; and
additional responsibilities that may be assigned as circumstances warrant.

Qualifications: The SED Waiver Facilitator position requires but not limited to:

a bachelor's degree in psychology, social work, psychiatric nursing or another related human services field or be equivalently qualified by work experience or a combination of work experience in the human services field and education with one year of experience substituting for one year of education.
reliable transportation; and a valid, unrestricted driver's license
prior computer experience and basic computer fluency;
strong interpersonal skills in working with diverse publics;
flexibility in assigned working hours;
a commitment to interdisciplinary and community mental health center approaches; and
an energetic and positive approach to the rapidly evolving changes and challenges of a complex work place.
must be able to achieve within one's first 6 months of employment a working competency in the management and use of the Center's electronic medical record (EMR) software as demonstrated by timely and accurate documentation of the clinical record
