Full Job Description

MDS NURSE – RN/LPN

Blaire House of Worcester is a family-owned and operated skilled nursing center that has been serving seniors for 50 years. Our 75-bed facility specializes in Short Term Care, Long Term Care and Rehabilitation. Our passion for providing exceptional care while ensuring that our seniors maintain a fulfilling quality of life is what sets us apart.

Job Summary:

Communicate, facilitate, instruct, direct, monitor, evaluate, and lead the assessment process.
Coordinate gathering and sharing of clinical information for the MDS and RAI process.
Monitor documentation to support payor claims.

Qualifications:

Education: Graduated from an accredited, approved school of nursing. Current Massachusetts registration with the State Board of Nurse Examiners.

Training and Experience:

Must have previous experience working on a nursing unit in a long-term care facility.
Required to understand all regulatory requirements in the area of documentation, including but not limited, to all state and federal regulations.
A necessity to understand OBRA, Medicare, Managed Care and case mix requirements.
Knowledge of MDS, RAI and Care Planning Process.
Must have excellent communication skills and possess team leadership.
Essential to be computer literate and the ability to meet deadlines is a must.

Salary: $90,000 - $95,000 annually

$4,000 Sign-On/Retention Bonus
