JOB SUMMARY: To coordinate and ensure the psychosocial, emotional, and physical needs of the residents are being met through the care and support of the staff, structured activities and other available networks, systems, caregiver or resources. To assist leading, under the supervision of the Administrator and Administrator Assistant, the coordination of housing and services offered at St. Otto’s Care Center for the Senior Population.
HOURS WORKED: Full Time, 64 hours per pay period. No Holidays or weekends.
I. QUALIFICATIONS:
a. A four (4) year Bachelor’s degree earned. Preferably in the subjects of Public Health, Social Work, Gerontology, Social Science, Administration, Management, or of similar topic.
b. Currently licensed as a Social Worker in the State of MN
c. Preference given to experience in the practice of social work in the long-term care setting.
d. Work effectively with minimal supervision, strong communication skills, and ability to lead a group of people in completing tasks.
e. Meet the physical job demands of the position
II. JOB DUTIES AND RESPONSIBILITIES:
PUBLIC RELATIONS:
a. Assist with facility marketing plans as delegated by Director of Social Services and campus Administrator.
b. Maintain weekly contact with discharge planners, case managers, and other sources for referrals as delegated by Director of Social Services.
c. Coordinate activities that will invite the community to visit (i.e. Community Service Programs; Estate Planning, Defensive Driving / Over 55 driving class, Care Giver Support Group)
d. Accepts inquires and provides tours to potential residents and their families
e. Maintains good relations with physicians, clinics, social worker, etc.
RESIDENTS:
a. Assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
b. Responsible for admissions and discharges with facility RN approval.
c. Participate in discharge planning, development and implementation of social care plans and resident assessments as delegated by Director of Social Services.
d. Involve the resident/family in planning objectives and goals for the resident.
e. Involve the resident/family in planning social service programs when possible
f. Assist in arranging transportation to other facilities when necessary.
g. Refer resident/families to appropriate social service agencies; Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident.
h. Provide each and every resident/family with the information necessary to explain their rights and responsibilities while staying at the facility.
i. Encourage resident/family members to attend quarterly care conferences.
j. Make supportive visits to residents to determine and monitor their psycho-social needs on an on-going basis.
k. Develop a written plan of care for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified; encourage the resident/family to participate in the development and review of his/her plan of care.
OPERATIONS
a. Plan, develop, organize, implement, evaluate, and direct the social services of the resident
b. Keep abreast of current federal and state regulations, as well as professional standards, and make any recommendations on changes in policies and procedures to the Administrator.
c. Review department policies and procedures, at least annually and participate in making recommended changes
d. Interview resident/families as necessary in a private setting
e. Perform administrative audits, such as completing necessary forms, reports, etc., and submitting such to the Administrator as required.
f. Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.
g. Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
h. Coordinate social service activities with other departments as necessary
i. Work with fellow departments and implement recommended changes as required.
j. Assist in collecting data for QA indicators
k. Implement recommendations from the Quality Assurance Committee as they relate to the Quality Assurance program
l. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility
m. Notify appropriate departments of new admissions, resident discharges/transfers, room changes, and resident deaths.
n. Complete/Review admission packet with resident/family upon admission; Therapeutic leave, Bill of Rights, payment rates, and other appropriate forms.
o. Coordinate any Resident’s room change; provide the necessary notice to the resident/family, roommate, and staff
p. Be involved in the formation of a facility Family Council and participate in Family Council meetings are requested.
q. Work with the facility’s Ombudsman as necessary and implement recommended changes as required.
r. Complete Pre-Admission Screening documents as required by County Social Services.
s. Completes PHQ-9, reviews care plans and MDS
t. Complete designated sections of the Minimum Data Set (MDS) and any necessary assessment during the Assessment Reference Date (ARD).
BUDGET
a. Ensure Payment source for resident’s stay
b. Work with business office on payment concerns, assuring resident has proper resources to meet financial obligations.
BUILDING
a. Request repairs for office equipment as necessary.
b. Check that resident’s personal belongings are labeled and note condition of such items upon placement in the building.
c. Provide the residents with the proper policies governing no-smoking/fire prevention.
d. Reports safety and environment hazards.
e.Responds appropriately to fire drills, codes, etc.
f. Maintains a clean and orderly environment.
EMPLOYEES
a. Maintain open communication with all departments to promote the best quality of care for residents/family.
b. Report to work on time, properly groomed according to departmental/facility dress code
c. Follow established ergonomics policies and procedures governing lifting techniques, repetitive tasks, and the use of safety equipment and supplies to prevent work-related injuries and illnesses.
