Position Summary
Aim Higher Supportive Services, Inc. is seeking compassionate, dependable, and motivated Community Support Associates (CSAs) and Adult, Family, or Youth Peer Support Specialists to provide Therapeutic Rehabilitation Program (TRP) services throughout the Louisville, Kentucky area.
The Therapeutic Rehabilitation Program (TRP) provides community-based rehabilitative services for adults diagnosed with Severe Mental Illness (SMI) and children and adolescents diagnosed with Severe Emotional Disability (SED). Services are delivered in participants' homes, schools, the community, and other approved settings to improve daily functioning, independent living skills, emotional regulation, and community integration.
Essential Job Responsibilities:
- Provide Therapeutic Rehabilitation Program (TRP) services in accordance with Kentucky Medicaid regulations, agency policies, and individualized treatment plans.
- Teach and reinforce independent living skills, social skills, communication, emotional regulation, problem-solving, conflict resolution, and coping strategies.
- Support participants in achieving treatment goals and increasing independence within their home and community.
- Assist participants in accessing healthcare, employment, education, housing, and other community resources.
- Facilitate community integration through structured recreational, educational, volunteer, vocational, and social outings.
- Transport participants safely to and from the TRP program, scheduled appointments, community outings, recreational activities, and their homes using agency vehicles, including passenger vans.
- Ensure the safe loading, unloading, and supervision of participants during transportation and community activities.
- Maintain accurate vehicle logs, mileage records, and complete routine vehicle safety inspections in accordance with agency policy.
- Follow all traffic laws, agency transportation policies, and safety procedures while operating agency vehicles.
- Provide direct supervision and support to participants during transportation and community-based activities.
- Collaborate with therapists, psychiatrists, schools, physicians, families, and other providers to coordinate services.
- Complete accurate and timely documentation, including progress notes, transportation logs, service documentation, and incident reports.
- Participate in staff meetings, supervision, required trainings, and quality improvement activities.
- Maintain participant confidentiality and comply with HIPAA, Kentucky Medicaid regulations, and agency policies.
- Utilize trauma-informed, person-centered, culturally responsive, and recovery-oriented approaches during all interactions.
Additional Responsibilities for Peer Support Specialists
- Utilize lived experience to provide recovery support, encouragement, mentoring, and advocacy.
- Model recovery principles and assist participants in developing wellness and recovery plans.
- Encourage self-advocacy, empowerment, and engagement in treatment and community activities.
- Assist families and caregivers in understanding recovery supports and available community resources.
Minimum Qualifications Community Support Associate (CSA)
- High School Diploma or GED required.
- Associate's or Bachelor's degree in Psychology, Social Work, Human Services, Counseling, Sociology, Criminal Justice, Education, or a related behavioral health field preferred.
- Experience working with adults with Serious Mental Illness (SMI), children with Severe Emotional Disability (SED), or individuals with behavioral health needs preferred.
Adult, Family, or Youth Peer Support Specialist
- Kentucky Certified Adult Peer Support Specialist, Family Peer Support Specialist, or Youth Peer Support Specialist, or eligible to obtain certification.
- Meets Kentucky Medicaid qualifications for Peer Support services.
- Demonstrated lived experience supporting recovery and resiliency.
Required Qualifications:
- Valid driver's license with a clean driving record.
- Minimum of one year of experience safely operating passenger vans or multi-passenger vehicles preferred.
- Ability and willingness to transport participants daily to and from the program, community outings, appointments, recreational activities, and their homes.
- Reliable transportation for work-related travel.
- Current automobile insurance.
- CPR and First Aid Certification (or ability to obtain upon hire).
- Successful completion of criminal background checks, abuse registry screenings, and drug screening.
Knowledge, Skills, and Abilities
- Knowledge of behavioral health rehabilitation and recovery principles.
- Strong interpersonal and communication skills.
- Ability to safely supervise participants in community settings.
- Excellent organizational and documentation skills.
- Ability to de-escalate behavioral situations and respond appropriately during crises.
- Ability to work independently while maintaining communication with supervisors.
- Basic computer proficiency and experience using electronic health records (EHRs).
Physical Requirements
- Frequent driving throughout Jefferson County and surrounding communities.
- Ability to safely operate passenger vans for extended periods.
- Ability to assist participants entering and exiting vehicles when necessary.
- Ability to stand, walk, bend, lift up to 25 pounds, and actively participate in community outings.
- Ability to respond effectively during emergencies involving participants.
Pay: $17.00 - $19.00 per hour
