Full Job Description

All Care is seeking a Hospice Admissions Nurse who will be responsible for evaluating patients for hospice eligibility, coordinating the admission process, and initiating high-quality, patient-centered end-of-life care in the home setting. This role requires strong clinical assessment skills, compassionate communication, and the ability to support patients and families during a critical transition.

Schedule: Monday-Friday

Work Location: On the road covering all service area

Essential Duties & Responsibilities:

Conduct comprehensive in-home assessments to determine hospice eligibility in accordance with Medicare and organizational guidelines
Complete admissions, including clinical documentation, consents, and care planning
Collaborate with physicians, case managers, and interdisciplinary team members to facilitate timely admissions
Educate patients and families on hospice philosophy, services, and expectations
Develop and initiate individualized plans of care
Ensure accurate and timely documentation in the electronic medical record (EMR)
Coordinate durable medical equipment (DME), medications, and initial services
Provide emotional support and guidance to patients and caregivers during the admission process
Maintain compliance with regulatory and agency standards
Participate in on-call rotation as required

Qualifications:

Valid driver’s license and reliable transportation
Active Massachusetts Registered Nurse (RN) license is required
Minimum of 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
Strong clinical assessment and symptom management skills
Knowledge of end-of-life care practices
Excellent communication and interpersonal skills
Ability to work independently and manage a caseload
Proficiency in using electronic health record (EHR) systems for documentation and communication
