Job Summary :
The Self Determination Coordinator oversees and coordinates the implementation of Person-Centered Planning and the principles of Self Determination in the lives of people supported by Community Living Services of Macomb County and assures that the Macomb County Community Mental Health’s (MCCMH) expectations and standards are met. The Self Determination Coordinator plays a key role in the recruitment and training and monitoring of Independent Supports Brokers/Network Providers. The Self-Determination Coordinator’s responsibilities include the mentoring of Independent Support Brokers/Network Providers in Person-Centered Planning, budget development and authorization, community outreach, obtaining and utilizing stakeholder input, assuring quality services and satisfaction with services while keeping the people supported by CLS and their needs the primary focus of all activities.
This position also functions as the Case Management Manager. The Case Management Manager is responsible for direct supervision and oversight of the CLS – Macomb County “Case Managers”. The Case Management Manager serves as a trainer, supervisor, mentor and advocate for Case Managers providing services to individuals with developmental disabilities. The essential function of the position is to facilitate the effective and efficient planning and implementation of the principles of Person-Centered Planning and Self-Determination, the Michigan Mental Health Code, CLS guiding principles, code of ethics and professional code of ethics
Reporting Relationship : Reports to the Community Living Services of Macomb County Director of Community Support
Essential Duties and Responsibilities:
- Coordinates the planning process and authorization of plans, budget development and management, and liaison and advocacy activities to pursue identified resources, entitlements, financial liability determinations and supports. Reviews Individual Plans of Service authored by case managers and Independent Supports Brokers/ Network Providers to assure that the MCCMH expectations and standards are met.
- Ensures the coordination and provision of emergency needs, required authorizations and releases, ORR, Grievance and Appeal and HIPAA notifications.
- Provides all stakeholders and enrollees with quality customer services and culturally respectful supports.
- In collaboration with the Case Managers and Independent Support Coordinator/ Network Providers, the person and their circle, develops an individual budget for each person supported, reflecting the services authorized in their Individual Plan of Service. Monitors individual and aggregate budgets via Revenue and Expense reports and finance tracking tools.
- Assures continuity of support to persons with developmental disabilities by reviewing required reports and summaries of service for funding sources. Reports individual and agency successes, outcomes and needs to funding sources and internal/external customers and stakeholders.
- Provides ongoing education to Case Managers and Independent Supports Brokers/ Network Providers on Self Determination including areas of individual budgeting, fiscal responsibility, money management, and budget authority. Conducts routine training sessions (i.e. Self-Determination, Individual Budgeting, etc.).
- Provides leadership and direction to Case Managers and Independent Supports Brokers/Network Providers, reinforcing organizational principles and setting the tone and expectation for quality service delivery.
- Utilizes databases, reports and tracking systems to monitor ongoing performance of case managers and Independent Supports Brokers/Network Providers to assure that the MCCMH expectations and standards are met. MCCMH standards are inclusive of, but not limited to, the timely submission of Individual Plans of Service, Annual Assessments, Crisis Plans, Progress Notes, and other job specific documentation.
- Schedules and facilitates Supervision meetings with Case Managers and Mentoring Meetings with Independent Support Brokers as needed and/ or directed by the Director of Community Supports and provides training and consultation to Independent Supports Brokers/Network Providers as needed should there be any concerns about meeting the ongoing expectation of CLS/MC’s funding sources.
- Educates and empowers persons served and families. Provides resources related to provider interviews and selection. Identifies gaps in service delivery/resources, and proactively acts to meet identified needs.
- Collaborates with traditional support systems to develop new and innovative methods of service provision that address gaps in service delivery and meet identified needs.
- Reviews and develops systems that assure adequate delivery of services to individuals with developmental disabilities. Meets with Network Providers, Community Mental Health Administrators, and various others as needed to ensure effective means of continuing quality services.
- Performs Quality Management functions. Develops and monitors systems to assure delivery of quality services. Prepares corrective action plans to address any patterns/trends that reveal need for improvement or areas of deficiency cited following internal or external review. Assists with the development, coordination, and monitoring of the CLS Compliance Plan.
- Completes Annual Assessments and Annual Person Centered Plans (as well as all associated documentation) in accordance with MCCMH, state and CLS quality standards.
- Responsible for Community Outreach activities including presentations to groups and individuals regarding Community Living Services and Self-Determination principles and practice.
- Assists Director with development and maintenance of the Handbook of Services.
- Participates in recruitment, development, and maintenance of a strong provider network.
- Participates in the development and implementation of policies and procedures.
- Participates on Agency Committees and planning groups specific to systems development.
- Recruits, interviews, selects, and assigns qualified staff as needed.
- Maintains confidentiality of personal and agency information.
- Performs special projects and other duties as assigned by supervisor.
- Chairs committee activity as appropriate. Attends and participates on committees as assigned.
- Accepts on-call responsibility.
Works adjusted hours as needed.

Qualifications and Requirements:
- Extensive scheduled and non-scheduled travel is required in executing essential duties and responsibilities of this position. Employees must provide their own transportation. Maintenance of a valid driver’s license, an acceptable driving record, and vehicle registration and insurance in compliance with State of Michigan requirements are all continuing conditions of employment. If driving a vehicle other than the one listed on file with the Human Resources Department, employee is responsible for ensuring the vehicle is registered, insured and meets all other State of Michigan motor vehicle requirements and providing documentation as requested.

Education and/or Experience:
- Minimum of Bachelor’s Degree in a Human Services area, and qualification as a Qualified Intellectual Disability Professional (QIDP) is required.
- Computer skills required, including word processing, spreadsheets, flowcharts, and use of internal and external e-mail. Knowledge of and Microsoft systems preferred.
- At least 3 years experience working with persons with developmental disabilities. Experience should demonstrate effectiveness in developing and implementing creative support solutions.
- Previous supervisory and or administrative experience is preferred.
Additional Knowledge/Skills:
Knowledge of the principles of Person-Centered Planning and, Self-Determination. A clear understanding of the roles and responsibilities of the case manager, Independent Support Broker and the Support Circle is necessary. Must have practical experience and demonstrated effectiveness in the development and monitoring of personal budgets. Good understanding of mental health functions and systems. Ability to use initiative, plan, organize, maintain systems, and meet deadlines (while paying attention to detail and maintaining a high level of accuracy). Ability to work well within a team atmosphere and maintain a positive, upbeat attitude. Desired attributes include creativity and a willingness to develop knowledge bases and share information with peers. Flexibility and a willingness to learn. Good interpersonal and negotiation skills with the ability to communicate effectively and establish positive professional relationships with both internal and external stakeholders.

Physical Demands:
- This job is essentially an office position with field work required. Physical tasks are minimal.
- Adjusted hours are required and scheduled and non-scheduled travel is a function of position. Travel to individuals’ homes, schools, hospitals, and other community locations as needed is required to execute job duties.
TRAINING:
- On the Job training will be provided.
- Completes all mandatory management training courses and participates in all other recommended training courses as approved by supervisor and contractually required.
- Completes a minimum of 24 hours of professional skill development activity each calendar year. If designated to work with children, completes 24 hours of child-specific training each calendar year.
