POSITION SUMMARY
The Money Follows the Person/Transition & Diversion Program (MFP/TDP) Program Director provides leadership, oversight, and administration for DCIL's MFP and TDP programs. This position ensures that program services, work plans, referrals, documentation, expenditures, and required reports comply with applicable state and federal requirements, contractual expectations, and DCIL policies while reflecting the organization's mission and the Independent Living Philosophy.
The Program Director supervises MFP/TDP staff; coordinates the assignment and monitoring of referrals; supports consumer transitions from institutional settings to community living; and ensures that services are person-centered, timely, accurately documented, and responsive to each individual's goals, choices, risks, and support needs. The position reviews consumer and program records in Therap and CIL Suite, conducts routine quality-assurance audits, monitors follow-up activities, and addresses documentation or service delivery concerns.
This position works collaboratively with consumers, families, guardians, transition coordinators, MFP/TDP administrators, HCBS representatives, community providers, Housing, DCIL leadership, and other partners. The Program Director facilitates meetings, supports staff training and development, promotes coordinated service delivery, and represents DCIL professionally in program, community, and interagency activities.
ESSENTIAL DUTIES & RESPONSIBILITIES
1. Develop and maintain an understanding of DCIL's mission, programs, services, policies, history, and the IL Philosophy, as well as MFP/TDP requirements, guidance, work plans, and contractual expectations.
2. Provide day-to-day leadership, direction, and supervision to MFP/TDP staff, including assigning duties, monitoring workloads, supporting case consultation, addressing performance concerns, and promoting consistent service delivery.
3. Coordinate the receipt, review, assignment, tracking, and follow-up of MFP and TDP referrals to ensure timely contact, appropriate action, and continuity of services.
4. Ensure consumers receive person centered support, advocacy, skills training, referrals, and transition related assistance that promote informed choice, community living, independence, and self-determination.
5. Review referral, transition, termination, Information and Referral (I&R), re-institutionalization, and ongoing case documentation in Therap and CIL Suite for accuracy, completeness, timeliness, and compliance.
6. Conduct and document quality-assurance audits at least monthly of required MFP/TDP records, including Transition Assessments, Transition Plans, Risk Assessments, backup plans, case notes, supporting documentation, and applicable database records.
7. Monitor consumer progress, service barriers, health and safety concerns, transition risks, and unmet needs; work with staff and program partners to resolve concerns and support successful community transitions.
8. Complete and submit accurate monthly, quarterly, annual, grant, and other required program reports within established deadlines, including reports requested by MFP/TDP administrators and DCIL leadership.
9. Monitor program statistics, outcomes, expenditures, service authorizations, and supporting documentation to ensure compliance with funding requirements and approved program activities.
10. Develop, review, update, and implement the annual MFP/TDP work plan, program procedures, goals, timelines, and quality-improvement activities in collaboration with the Executive Director and appropriate DCIL staff.
11. Facilitate and participate in MFP/HCBS territory meetings, program staffing, transition meetings, and other meetings with MFP/TDP administrators, staff, consumers, and community partners.
12. Develop and maintain effective working relationships with state agencies, HCBS representatives, transition coordinators, service providers, housing resources, community organizations, and other partners involved in supporting community living.
13. Provide clear and timely communication to consumers, families, guardians, staff, and partners regarding referrals, program processes, services, documentation requirements, and follow-up responsibilities.
14. Assist with recruiting, interviewing, hiring, onboarding, and training MFP/TDP staff; coordinate initial and ongoing training to ensure staff understand program requirements, consumer documentation, data entry, confidentiality, and DCIL services.
15. Conduct regular supervision meetings, probationary reviews, and annual performance evaluations in coordination with the Executive Director, and provide timely coaching, feedback, and corrective direction when needed.
16. Collaborate with the Executive Director, IL Program Director, Board of Directors, and other DCIL staff on strategic planning, policy development, outreach, education, and organizational goals.
17. Work cooperatively with staff in both DCIL offices to promote consistency in service delivery, referrals, documentation, policies, procedures, outreach, education, and community engagement.
18. Represent DCIL and promote MFP/TDP services through professional communication, outreach, presentations, community events, interagency collaboration, and systems advocacy activities.
19. Maintain strict confidentiality, safeguard consumer and agency information, and comply with all applicable laws, program requirements, and DCIL policies and procedures.
20. Travel as needed to meet consumer and program needs, attend meetings and trainings, monitor services, and carry out assigned responsibilities throughout DCIL's service area.
21. Perform other reasonable duties as assigned by the Executive Director.

2. Maintain current knowledge of disability services, community resources, housing and transition supports, applicable program guidance, and changes affecting MFP/TDP services.

SUPERVISORY RELATIONSHIPS
This position is supervised by the Executive Director.
SUPERVISORY AUTHORITY
The MFP/TDP Program Director provides direct supervision to MFP/TDP program staff and, when assigned, volunteers or interns.

2. Understanding of the Independent Living Philosophy, disability rights movement, community integration, and the Americans with Disabilities Act (ADA).

6. Ability to maintain strict confidentiality and handle sensitive consumer, personnel, and program information appropriately.
QUALIFICATIONS
1. Bachelor's degree preferred in Human Services, Social Work, Rehabilitation, Public Administration, Business Administration, or related field; or a combination of relevant education and experience that demonstrates the ability to perform the essential functions of the position.
2. A minimum of three (3) years of related experience in disability services, Independent Living, human services, program administration, or a related area, including leadership, supervisory, or program management responsibilities.
3. Experience working with people with disabilities and knowledge of the Independent Living Philosophy, disability rights movement, person-centered practices, and community-based services is strongly preferred.
4. Strong organizational, interpersonal, oral, and written communication skills, with the ability to manage multiple priorities, lead meetings, meet deadlines, and maintain accurate records.
5. Ability to work independently, exercise sound judgment, maintain confidentiality, and communicate effectively with individuals from diverse backgrounds and socioeconomic groups.
6. Proficiency with Microsoft Office and the ability to learn and use Therap, CIL Suite, and other required program or reporting systems.
7. Ability to travel as required. A valid driver's license and proof of automobile liability insurance are required.
SPECIAL CONDITIONS
1. Occasional work outside of standard business hours (8:00am-5:00pm) may be required.
2. Subject to a 90 day or 180 day probationary period.
