Position Overview
We are seeking a dedicated Brown County Case Manager to deliver high-level team oversight, system coordination, and stakeholder management across our regional behavioral health and rehabilitative programs. Operating within a person-centered, trauma-informed care framework, this role balances regulatory compliance with our creative, non-clinical philosophy.
Reporting directly to the Chief Behavioral Health Officer (CBHO) and working closely with HR and Quality Assurance, you will serve as the central operational hub; connecting frontline providers, youth and adult consumers, families, and Brown County facilitators (including Comprehensive Community Services teams) to ensure safe, compliant, and impactful care.
Key Responsibilities
Team Oversight & Staff Development
- Scheduling & Coverage: Audit daily staff schedules to optimize service delivery; rapidly reconcile provider and consumer cancellations using established platforms and waitlists.
- Performance Coaching: Maintain real-time provider logs for positive feedback and coaching opportunities; escalate high-priority operational or clinical concerns to HR and the CBHO.
- Staff Leadership: Facilitate weekly and monthly staff meetings; conduct regular check-ins; lead monthly scheduling blocks.
- On-Call Rotation: Serve as the designated on-call manager for one full weekend per month to navigate operational or consumer crises.
- On-Call Management: Participate in the designated remote on-call rotation (one weekend/weeknight per month) to guide staff through de-escalation or operational alerts via chat and phone.
System Operations & Quality Assurance
- Process Improvement: Partner with the CBHO to design, document, and implement streamlined operational processes for Brown County.
- Onboarding & Consumer Provisioning: Manage incoming Brown County referrals; build and maintain comprehensive Consumer Demographic profiles (including trauma histories, activity preferences, and critical safety information).
- Budgeting & Benchmarks: Track regional activity and expense spreadsheets; ensure providers and programs consistently meet performance and financial benchmarks.
Stakeholder & Family Collaboration
- Multidisciplinary Coordination: Lead employee representation in monthly county team meetings; facilitate pre-meeting briefings, track consumer goal progress, and generate comprehensive Service Notes for QA review.
- Family Engagement: Act as the primary escalation point for families regarding schedule updates, policy notifications, and emergency alerts using preferred communication channels.
Qualifications
Required:
- Valid Driver’s License with a clean driving record, plus access to a reliable, insured personal vehicle (mileage reimbursement provided).
- Proficiency with client management, scheduling databases, and shared drive platforms.
Preferred:
- Education: Bachelor’s or Master’s degree in Social Work, Psychology, Sociology, Human Services, Special Education, or a related field.
- Experience: Background as a Case Manager, Service Coordinator, or Team Lead within Comprehensive Community Services (CCS), Youth Justice (YJ), or behavioral health programs.
Pay: $39,500.00 - $45,000.00 per year
