What You'll Be Doing
- Complete in-home Occupational Therapy evaluations.
- Develop individualized plans of care.
- Assess ADLs, functional abilities, safety, and independence.
- Provide interventions focused on functional performance, strength, coordination, and motor skills.
- Address bathing, dressing, grooming, meal preparation, and daily routines.
- Recommend adaptive equipment and environmental modifications.
- Educate patients and caregivers on compensatory and safety strategies.
- Monitor progress and adjust treatment plans.
- Collaborate with PTs, nurses, physicians, and other interdisciplinary clinicians.
- Complete accurate and timely clinical documentation.
Bring Your OT Experience With You
We're interested in Occupational Therapists from a variety of clinical backgrounds, including Home Health, Outpatient Therapy, Acute Care, Inpatient Rehabilitation, Skilled Nursing, Transitional Care, Senior Living, and Long-Term Care.
Previous home health experience is preferred, but we're equally interested in experienced clinicians who have the independence and organization needed to successfully transition into the field.
What We're Looking For
- Active Minnesota Occupational Therapist license
- OTR/L
- Valid driver's license and reliable transportation
- Strong clinical reasoning and functional assessment skills
- Ability to independently organize and manage a field-based caseload
- Excellent patient and caregiver communication
- Home health experience preferred
- Experience with geriatric or medically complex patients welcomed
- EMR experience preferred
