Summary/Objective

Join our alternative learning placement school, Growing Minds Education, as a Case Manager and make a lasting impact on the lives of students who need it most. In this dynamic educational environment, you will work directly with youth of all ages to help them overcome barriers and achieve both personal and academic success.

This role gives you the unique opportunity to actively apply and expand your clinical skills every day. You will provide holistic, individualized support to students while facilitating engaging group sessions. Using targeted crisis de-escalation techniques, you will help maintain a safe, nurturing space where every student feels valued and understood.

You will not do this work alone. Partnering closely with educators, families, and community resources, you will be a vital part of a highly collaborative team dedicated to student well-being. If you are a mission-driven individual who is passionate about advocating for at-risk youth, this role allows you to be the turning point in their educational journey.

Essential Duties and Responsibilities

Provide school-based and community-based support, advocacy, and other daily activities to assigned clients
Required to transport youth when necessary
Maintain set schedule M-F 7:30 am - 3:30 pm , no weekends or holidays
Facilitates group activities and development, assists with activities appropriate for group’s level of functioning
Assists with providing crisis care and de-escalation
Maintain required direct care interaction including all necessary documenting and reporting, attendance of meetings, trainings, and all standard measurable metrics and agency standards set for role
Maintain Client Interaction of 65% of worked hours
Establishes and maintains positive working relationship internally and externally
Communicate with agency staff in a professional manner
Maintains quality results by following agency standards-attend clinical supervision, manage caseload effectively, manage and maintain treatment plans, adhere to on time and quality documentation standards
Maintains continuity of work operations by documenting and communicating actions, irregularities, and continuing needs
Maintains patient confidence by keeping patient records, information confidential per HIPAA
Serves and protects the agency by adhering to professional standards, agency policies and procedures, federal, state, and local requirements, and CARF standards
Perform other duties, as assigned, to the need of the agency deemed necessary and appropriate

Qualifications, Education and Experience

Required:

High School Diploma required
At least 3 years of case management or managing of children's behaviors

Bachelor or Master’s degree in a human services field (SW, Criminal Justice, Psychology, Sociology, Human Development, Counseling, Education, etc.)

Must have valid Driver License with acceptable driving record defined by the agency commercial insurance carrier. Must not have a criminal record that would exclude you from working in a child-serving environment. Must also be able to work well in a fast-paced, at times high-pressure environment

Preferred:

· At least three (1) year of experience with case management or direct services with at-risk youth

Necessary Knowledge, Skills and Abilities

· Excellent time management, organization, attention to detail, quality focus, professionalism, productivity, thoroughness, coordination, deadline-oriented, internal communications, reporting skills.

Tools and Equipment Used

Agency issued P.C. with necessary Microsoft Suite applications, including work processing software, copy machine, and fax machine as necessary.
