CW Solutions has an immediate opening for a split role of Comprehensive Community Services (CCS) Service Provider and ROUTE Case Manager / Service Navigator. Please note that while this position is 50% ROUTE Case Manager / 50% CCS Service Provider position, the position will be modified to 100% ROUTE Case Manager through May 2027 to meet ROUTE’s current programmatic needs. This position is anticipated to have two primary office locations through May 2027 - Wisconsin Rapids (PEC) and Wausau, with travel throughout Portage, Adams, Wood, and Marathon Counties. The individual selected for this position will be responsible for their travel and time for their commute to both offices (Wausau and Wisconsin Rapids). All work-related travel completed during the workday will be reimbursed following our standard mileage reimbursement process and rate.
According to the Wisconsin Department of Health Services (DHS), “CCS is a program that helps individuals of all ages live their best life by providing support that addresses their unique needs related to mental health and substance use. CCS is intended to assist individuals who are in need of care outside of inpatient settings, but who may have ongoing needs that if left unaddressed, could result in hospitalization during times of crisis.” This position works with youth and adults, under the direction of a CCS plan of care established by the CCS team with Wood County Human Services. The age of clients served spans the lifetime, from early childhood to aging adults. The CCS service provider is one member of a larger team providing wrap-around services for clients in need of support.
ROUTE (Resources and Opportunities to Uplift Trafficking Experiences) is a program through the Department of Children and Families that focuses on providing case management, services and support to youth who have experienced sex trafficking. The ROUTE Case Manager is to provide navigation of wraparound community services and resources to empower youth self-sufficiency, decision-making, and empowerment. Travel will be required throughout Adams, Portage and Wood Counties. This spilt position requires flexibility of schedule in response to the needs of the clients and families, including evening hours. Weekend hours are not required, although they may be available if the preference of the employee and needs of the client align. Although this is a full-time position, weekly hours may vary based upon the current number of clients being served. This position is eligible for all full-time employee benefits. The position is guaranteed a minimum 30 hours per week.
General Functions:
Comprehensive Service Provider Functions:
The CCS Service Provider provides services within a client-centric system of care, as described in the client’s Individual Recovery Plan (IRP). The primary services provided for clients include individual skill development, individual psychoeducation, and family psychoeducation interventions.
To be effective, the service provider will work with clients in their homes and communities. Activities often include interactions with client families and schools.
The service provider is responsible for mentorship, technique-building and facilitating strengths-based intervention-focused activities, working collaboratively with the CCS team to assist clients in developing skills to successfully manage their mental health condition while engaging in their family, school and social life.
ROUTE Case Manager / Navigator Functions:
The part-time ROUTE Case Manager provides case management and navigation of services to youth who have experienced sex trafficking in their adolescence and/or early adulthood.
As part of their role, they will conduct needs and goals assessment with youth to determine their self-identified goals related to employment, education, housing, wellbeing, and permanent connections. This position is responsible for mentorship, technique-building, and increasing resiliency with youth.
Essential Functions:
The following duties are normal for this split position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned. Functions are always customized to each client’s Individual Recovery Plan (CCS) or ROUTE youth.
Comprehensive Service Provider Skills:
1. Provide skills training and education through modeling, monitoring, mentoring, supervision, assistance, cuing and other methods that best match the clients’ needs and learning style.
2. Conduct individual skills development and enhancement for clients. This may include:
- Problem-solving and decision making.
- Self-regulation and conflict resolution.
- Behavior awareness and behavior change.
- Improving integration and interaction with family, friends, school and community.
- Daily living skills.
- Develop, maintain and follow goal plan for each client served.
3. Conduct individual and/or family psycho-education. This may include:
- Skills training.
- Social and emotional support for dealing with mental health and/or substance abuse issues.
- Ongoing guidance about managing and coping with mental health and/or substance abuse issues.
- Education and resources about the client’s mental health and or substance abuse issues.
- Set realistic expectations and goals based on each client’s strengths, talents, abilities and needs.
ROUTE Case Manager / Navigator Skills:
1. Partner with agencies and organizations in Adams, Wood, and Portage Counties to identify and work with youth who have experienced sex trafficking and serve on multidisciplinary teams to address sex trafficking cases, trends and growth areas within communities.
2. Conduct outreach to youth who have been identified as eligible for the program to enroll in program services. Outreach efforts include:
- Face-to-face outreach efforts in local office or in a community setting.
- Use of phone, text, social media, letters, post cards and other creative outreach efforts to reach youth about the services available through the program.
- Repeat efforts to maximize opportunities for youth to engage in services.
- Develop and maintain rapport with youth to sustain engagement in program services.
Skills for Both:
1. Maintain accurate and detailed accounts of all activities in assigned systems. Entries must be completed daily, no later than one business day after any action or interaction that requires documentation.
Education: Required
A Bachelor’s Degree in Social Work, Sociology, Human Services, Health and Wellness or related Human Service field that provides the necessary skills to meet the expectations of the position
Experience: Required
At least one year experience serving individuals with mental health or behavioral challenges or in case management serving individuals or families.
Transportation: Required
Must have a valid driver’s license, hold current vehicle insurance and have access to reliable transportation to travel to various locations to serve program participants in the community throughout Wood County. Mileage reimbursement is provided for all business-related travel outside of the assigned office.
