CURRENT OPPORTUNITY TO JOIN OUR TEAM:
The Wood County Human Services Department, has an immediate opening for a Registered Nurse - Comprehensive Community Services (CCS Community Support Program (CSP). CCS/CSP Nurses work as part of a multidisciplinary team providing treatment and support to individuals with mental illness and substance use disorders in the community. Registered nurses provide medication monitoring, symptom management and evaluation, direct support to Psychiatrist and APNP’s, case management, physical health assessment and other supportive activities. Contact with clients occurs both in the office and in other community settings.
RESPONSIBILITIES
A. ESSENTIAL FUNCTIONS
1. Provides nursing services and education regarding potential side effects of prescribed medication to consumers. Services include injections; medication setup; blood pressure readings, weight monitoring, and overall health assessment as part of a psychiatric examination; phoning in a medication refill or new prescription as ordered by the prescriber to the consumer’s pharmacy; ordering and monitoring of lab work related to prescriber ordered medications. Education includes explaining need for medication, how it works, and potential side effects. Also provides linkage to medical community for clients and staff when necessary.
2. Work collaboratively with clients, their families, guardians and recovery teams in assessment and in developing integrated service plans. Provide linkage for collaboration between clients, contracted service providers, physicians and other community professionals. Use strategies including strength-based assessment to establish hope, facilitate empowerment, and promote stabilization in the community to achieve optimal independence and recovery.
3. Maintain documentation as mandated by Wisconsin Medical Assistance Guidelines, Wisconsin Nurse Practice Act, and the department’s Nursing Standard of Care policies as well as parameters established within DHS 36 and DHS 63 and Medical Assistance guidelines, complete required documentation, to include but not limited to assessments, treatment plans, contact notes, functional screen eligibility, and training and supervision logs. Review vendor monthly invoices and documentation to assure accuracy of billing, and compliance with recording requirements
4. Provide recovery-based individualized case management services to children and adults with mental health and substance use disorders as assigned. Work collaboratively with consumers and their recovery teams in assessment and in developing integrated service plans. Provide linkage and collaborate with contracted service providers, problem solve, advocate, monitor and follow up on services delivered to the consumer.
5. Provide CSP services to clients as part of the CSP Treatment Team. Medication set up, medication delivery to clients, consultation with team members, team staffing, coordination of medical care, complete health histories, assist clients with accessing medical care.
6. Assist non-nursing case managers, as requested, for medical needs of their clients. Conduct physical health assessments as required for medical triage and facilitate access to medical care as appropriate. Promote access to annual physical/dental/eye preventative care appointments. Provide linkage and timely communication to the medical community which may include transportation and participation in clinic visits to facilitate coordination of care.
7. Organize health promotion/disease prevention activities for clients, researching opportunities and promoting client participation. Assist clients to set personal goals, documenting individual and program outcomes.
8. Introduce clients to community and individual opportunities that foster interest in life-long healthy lifestyles. Activities will be individual or group oriented, and promote:
- Daily physical exercise
- Healthy nutritional choices
- Participation in community wellness programs or support groups
- Participation in risk reduction programs
- Compliance with medical recommendations for chronic disease management.
9. Provides telephone exchanges with or about patients related to assessment and management services, including prescription provision and laboratory orders
10. Know and practice the safety policies of the County. Perform all job tasks in a safe and prescribed manner.
11. Must be familiar with and exemplify the Wood County Core Values and serve with courtesy and respect in accordance with the Wood County Code of Conduct.
B. OTHER DUTIES
1. Responds to medical emergencies in the building.
2. Participate in continuing education via conferences, web-based and in-service training to maintain current nursing best practice standards.
3. Provide consultation and education about mental health issues to service agencies, business and community organizations to improve understanding of the mental health delivery system, promote personal mental well-being, and reduce stigma of individuals with mental illness and/or substance abuse issues.
4. Triages patient calls for the Psychiatrist and/or Nurse Practitioner. Assesses need, and takes appropriate action.
5. Provides emergency screen/assessment of individuals who walk into the clinic looking for or in need of services. May include brief intervention to calm an individual, assessment of immediate need to see the Psychiatrist or clinicians.
6. Assists with provision of telehealth as scheduled for coordination and troubleshooting in keeping with requirements for State Certification under DHS 75, 34, 63, 36 , and 61.91.
7. Perform other duties as assigned.
EXPERIENCE, TRAINING, QUALIFICATIONS
Bachelor’s Degree in Nursing or associated health field from an accredited college and two years of experience or an equivalent amount of nursing education and experience (an Associate Degree in Nursing (two year degree) and four years’ experience) and State of Wisconsin licensure as a Registered Nurse is required. Must have an understanding of techniques and skills useful in assisting individuals with a chronic mental illness. Position requires the ability to work effectively with a variety of other professionals and manage time effectively. Ability to understand and follow directions (department manuals, etc.), and reading and writing reports is necessary. Ability to communicate orally and in writing with individuals and groups. This position may require travel to and from off-campus sites. Possession of a valid driver’s license, as well as a licensed and insured automobile is required; or other means of reliable transportation immediately available to individual. Experience working with the chronically mentally ill in the community is desirable.
