Full Job Description

We are seeking an experienced HCS Case Manager to join our team—someone who brings confidence, professionalism and coordinating high-quality, person-centered services.

This Role Is for Experienced Professionals

This position is best suited for seasoned case managers who:

Are comfortable managing complex caseloads
Thrive in fast-paced, high-accountability environments
Take pride in advocating for individuals and building strong teams

You will provide comprehensive case management services to the individuals supported in our Foster Family Community. This is a leadership role with real influence over outcomes and quality of care.

Key Responsibilities

Ensure all services are delivered in full compliance with HCS Program Provider Survey and Certification Standards and billing guidelines
Coordinate and monitor residential, vocational, and community-based services in alignment with each individual’s Individual Plan (IP) and IPC
Develop and implement individualized strategies that promote independence, skill development, and reduced support needs over time
Supervise, coach, and support families to maintain high standards of care
Provide ongoing encouragement, guidance, and performance support
Serve as a strong advocate for individuals, ensuring dignity, choice, and self-direction

Qualifications

High school diploma or GED with a minimum of 2 years of experience in social services, behavioral health, human services, or a related field
Bachelor’s degree in Human Services or related field preferred
Prior experience in HCS, I/DD services, or a closely related field strongly preferred
