Job Summary
As a Registered Nurse in home care, you will be responsible for assessing clients' health needs, developing personalized care plans, providing clinical oversight, and supporting the delivery of high-quality in-home care services. The RN will work closely with clients, families, caregivers, and the care team to promote client safety, independence, dignity, and quality of life.
The RN will also provide supervision and oversight of PCA/CNA caregivers, including conducting supervisory visits and completing caregiver competency evaluations to ensure caregivers are performing assigned duties safely and in accordance with agency policies, client care plans, and applicable regulations.
Responsibilities
- Conduct initial assessments of clients' health status, functional needs, and care requirements.
- Develop and maintain comprehensive care plans tailored to each client's specific needs, preferences, and goals.
- Monitor and evaluate clients' progress and care needs, updating care plans as appropriate.
- Provide clinical oversight and supervision of PCA/CNA/Caregivers providing services in the home.
- Conduct supervisory visits to observe caregiver performance, client condition, safety, and adherence to the established plan of care.
- Complete competency evaluations for PCA/CNA/Caregivers to verify their ability to safely and appropriately perform assigned care tasks.
- Identify caregiver training needs and provide or coordinate education and guidance as appropriate.
- Ensure caregivers understand and follow client-specific care instructions, agency policies, and applicable regulatory requirements.
- Document supervisory visits, assessments, competency evaluations, care plan updates, and other required clinical documentation accurately and timely.
- Communicate changes in a client's condition, care needs, or safety concerns to the appropriate members of the care team and client's representative as applicable.
- Provide education and support to clients, families, and caregivers regarding safety, care needs, and appropriate use of available resources.
- Promote client rights, dignity, independence, and autonomy in the delivery of care.
- Ensure compliance with safety protocols and infection control measures to maintain a safe environment for clients and caregivers.
- Participate in quality assurance and compliance activities related to caregiver supervision, competency, and client care.
- Participate in ongoing training and professional development activities to remain current with nursing practice, home care standards, and applicable regulations.
- Maintain confidentiality and comply with HIPAA and agency privacy policies.
- Perform other nursing and supervisory duties within the scope of the position, agency policies, and applicable laws and regulations.
Qualifications
- Valid RN license in the state of North Carolina.
- Minimum of 2 years of nursing experience, preferably in home care, geriatrics, community health, or a related setting.
- Experience supervising or evaluating nursing assistants, personal care aides, or caregivers preferred.
- Excellent clinical assessment and critical-thinking skills.
- Strong communication, leadership, and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and client-centered approach to care.
- Proficiency in using electronic health record (EHR) systems and other relevant healthcare technologies.
- Reliable transportation and willingness to travel to clients' homes.
- CPR certification may be required.
- Bachelor of Science in Nursing (BSN) preferred.
Working Conditions
- Flexible scheduling, including occasional evenings and weekends, based on client and agency needs.
- Frequent travel to clients' homes within the assigned service area.
- Exposure to a variety of home environments, including limited space, pets, and other household factors.
- Physical demands may include bending, standing, walking, and assisting with or observing transfers and other activities of daily living.
- Ability to safely perform supervisory and competency evaluation duties in the client's home.
