Social Worker (MSW)

Region VII Area Agency on Aging/Sunrise PACE is accepting applications for a highly motivated Social Worker (MSW). We are a growing non-profit organization based in Fort Gratiot, MI. The Company: Region VII Area Agency on Aging is a non-profit organization that helps seniors, adults with disabilities and their families have greater independence in their homes and communities and is known for the great care we take with clients and employees alike. We believe in advocating, planning, developing, and supporting a comprehensive system of quality care and services designed to achieve the optimum level of health, well-being, and independence of people as they age. Sunrise PACE is a Program of All-Inclusive Care for the Elderly located in Fort Gratiot, MI. An alternative to nursing home living, services provided by Sunrise PACE enable those 55 years of age or older to age in place in their home or community setting of choice.

Purpose of the position

The Social Worker performs a comprehensive psychosocial assessment of PACE (and possibly other program) participants to identify program eligibility, and supportive service needs. In cooperation with the other members of the Interdisciplinary Team, this position is also responsible for the development of participant care plans, arrangement for health and health related services prescribed under the care plan, and ongoing monitoring and reassessment of participants. Has as his/her primary duty, the performance of work requiring advanced knowledge in a field of science or learning customarily acquired by a prolonged course of specialized instruction.

Requirements for this position

This position requires a degree of Masters of Social Work with experience in community health programs preferred.
A minimum of two years of experience in a human services agency, with preference given to those with prior experience working with the elderly and/or disabled.
A working knowledge of third party reimbursement mechanisms and an understanding of the social service delivery system.
Must be able to read and interpret documents such as Medicaid policies and interpret supporting documentation.
Working knowledge of public programs, including local community health and social service network required.
Ability to become and maintain MMAP Certification (Medicare Medicaid Assistance Program).
Ability to communicate with elderly and disabled participants on a one-to-one basis, analytic and organizational abilities, interviewing and listening skills, assertiveness skills, computer skills (for initial information set-up of services & on-going monitoring), and ability to work effectively as a member of an interdisciplinary team.
Valid CPR/AED certification.
