Wellness Coordinator - Assisted Living & Memory Care

Astoria Park Senior Living | Pasadena, CA

Pay: $31.00-$33.00 per hour
Job Type: Full-time
Schedule: Sunday through Thursday, 12:00 PM-8:00 PM

Join Our Wellness Team

Astoria Park Senior Living is seeking a compassionate and dependable Wellness Coordinator to join our assisted living and memory care team in Pasadena.

This hands-on position works closely with the Wellness Director to support residents, families, caregivers, and medication technicians and help coordinate day-to-day resident services.

Candidates with previous experience in assisted living, memory care, senior living, or a related healthcare setting are encouraged to apply. Previous care coordination, medication management, or team leadership experience is preferred.

Responsibilities

Support the Wellness Director with day-to-day resident services.
Provide guidance and support to caregivers and medication technicians.
Help ensure resident services are consistent with established service plans.
Maintain accurate resident documentation as assigned.
Communicate resident needs, concerns, and observed changes to the appropriate team member.
Communicate professionally and compassionately with residents and families.
Assist with new resident move-ins and transitions.
Help coordinate communication among the Wellness team and other community departments.
Assist with staff training, coaching, and day-to-day problem solving as directed.
Follow community policies, procedures, and established resident care practices.
Participate in meetings and training as requested.
Perform other duties as assigned.

Qualifications

Previous experience in assisted living, memory care, senior living, long-term care, or a related healthcare setting preferred.
Experience working with older adults preferred.
Memory care or dementia care experience is a plus.
Some knowledge of assisted living practices and regulations preferred.
Previous lead, supervisory, medication technician, or care coordination experience preferred.
Strong communication, organizational, and interpersonal skills.
Ability to provide guidance and support to care team members.
Basic computer and documentation skills.
Dependable, professional, compassionate, and resident-focused.

Candidates with relevant senior living, resident care, memory care, or healthcare experience are encouraged to apply.
