Full Job Description

Position Overview

The Arc of the South Shore has been awarded a contract from the Department of Developmental Services (DDS) for a new Home-Based Residential Supports (HBRS) Pilot. The Arc is supporting individuals with significant medical, behavioral, and developmental support needs who are living in their family homes as an alternative to a 24/7 residential placement.

Reporting to the Community Supports Director, the Community Supports Nurse provides clinical nursing oversight, consultation, and direct nursing support to individuals and families participating in the HBRS Pilot. The nurse works collaboratively with individuals, families, and interdisciplinary team members to promote health, safety, and medical stability while supporting community inclusion, independence, and quality of life.

Using a person-centered, trauma-informed, and holistic approach, the Community Supports Nurse ensures that medical needs are appropriately identified, monitored, and addressed across home and community settings. This role also provides nursing consultation across other Community Supports and Residential programs as needed to support safe, coordinated, and high-quality care.

This is a part-time position. The HBRS program is contract funded and contingent upon continued support from DDS.

Key Responsibilities

Clinical Nursing Support & Health Oversight

Provide nursing assessment, monitoring, and consultation to individuals receiving HBRS services, with a focus on overall health status, medical stability, and preventive care.
Review and interpret health-related information, including diagnoses, medications, treatment plans, and physician orders, to support safe service delivery.
Identify health concerns, changes in condition, and emerging risks, and coordinate appropriate follow-up with families, providers, and clinical team members.
Support individuals with complex medical needs in collaboration with physicians, specialists, and other healthcare providers.

Care Coordination & Interdisciplinary Collaboration

Collaborate with families, Community Supports staff, nursing staff, DDS representatives, physicians, therapists, schools, and other providers to ensure coordinated and consistent care.
Participate in team meetings, case reviews, and interdisciplinary planning to support Individual Support Plan (ISP) goals and health-related objectives.
Provide clinical recommendations to support service planning, risk management, and long-term stability in the family home setting.
Serve as a nursing resource across Community Supports and Residential programs as needed.

Medication Administration & Health Management

Provide oversight and guidance related to medication administration, medication safety, and medication documentation practices.
Review medication orders, monitor for side effects or contraindications, and communicate concerns to prescribers and team members.
Support staff and families in understanding medication regimens, health routines, and prescribed treatments.
Ensure compliance with applicable nursing standards, DDS requirements, and agency policies.

Health Education & Family Support

Provide education and coaching to families and caregivers regarding health conditions, chronic disease management, preventative care, and basic nursing supports.
Support families in recognizing changes in health status and understanding when to seek medical attention.
Promote health literacy, empowerment, and informed decision-making within the family system.

Documentation, Compliance & Professional Responsibilities

Maintain accurate, timely, and compliant nursing documentation in accordance with agency, DDS, and licensing standards.
Document nursing assessments, interventions, recommendations, and health-related progress in service records and clinical notes.
Ensure confidentiality of protected health information and adherence to HIPAA and all applicable regulations.
Exercise sound clinical judgment and adhere to all nursing practice standards, ethical guidelines, and agency policies.

Program Support & Consultation

Provide general nursing consultation to other Community Supports and Residential programs as needed.
Assist in identifying health-related training needs for staff and support the development of health and safety competencies.
Contribute to quality improvement initiatives and program development efforts related to health and clinical care.

Education, Licensure & Training

Current Licensed Practical Nurse (LPN) required; Registered Nurse (RN) preferred.
Minimum of 3 years of nursing experience preferred, ideally in developmental disabilities, home health, community health, or similar settings.
Experience supporting individuals with intellectual and/or developmental disabilities and complex medical needs preferred.
Knowledge of DDS-funded services is preferred.
Demonstrated ability to effectively engage and support individuals from diverse backgrounds, including across protected characteristics such as race, color, ethnicity, culture, language, gender identity, sexual orientation, age, disability, religion, national origin, and genetic information, in accordance with applicable federal and Massachusetts anti-discrimination laws.
Current First Aid and CPR certification required. Must have valid driver's license, satisfactory driving record, and acceptable background checks. Must have access to a reliable and insured vehicle and be willing to travel across Southeastern MA.
Must be comfortable working in family homes and community-based settings.

Compensation

Job Type: Part-Time

Schedule: Approximately 10 hours per week. Schedule is flexible and will be based on program needs.

Compensation: $35-40/hour (commensurate with experience, education, and qualifications)

Location: Southeastern MA; Remote with occasional travel for home visits.

Pay: $35.00 - $40.00 per hour
