Job Summary

Join our dynamic home care team as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) providing personalized, compassionate care to clients in their homes. This role offers an exciting opportunity to make a meaningful difference in the lives of individuals with diverse medical needs, including those with developmental disabilities, dementia, or recovering from acute illnesses. You will deliver skilled nursing services, coordinate care plans, and support clients’ independence and well-being in a flexible, community-based setting. Your expertise will help ensure safe, effective, and holistic patient care while fostering strong caregiver communication and adherence to regulatory standards.

Duties

Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individualized care plans.
Administer medications via injections, IV infusions, catheter care, and other methods while ensuring proper documentation and adherence to HIPAA confidentiality policies.
Manage complex medical procedures such as ventilator support, tube feeding, sterile processing, and wound suturing within the home environment.
Coordinate discharge planning and case management for clients transitioning from hospitals or skilled nursing facilities to ensure continuity of care.
Utilize electronic health record (EHR) systems like Epic, Cerner, or Athenahealth for accurate medical documentation, medication administration records, and diagnostic evaluation reports.
Provide specialized care for patients with Alzheimer’s disease, dementia, traumatic brain injuries (Level I trauma center experience), infectious diseases, or requiring hospice services.
Educate caregivers and family members on medication administration, first aid techniques, infection control practices, and physiologic management strategies such as airway management and pain control.

Experience

Proven experience working with geriatric patients in long-term care facilities such as nursing homes or assisted living communities.
Critical care experience including ICU or emergency room settings with exposure to acute pain management and trauma response.
Demonstrated proficiency in EMR/EHR systems like Epic or eClinicalWorks; familiarity with ICD-9/10 coding and CPT coding is preferred.
Background in home & community care environments with knowledge of long-term care regulations and home safety protocols.
Skilled in patient assessment techniques involving vital signs monitoring, specimen collection/processing, venipuncture, phlebotomy, and sterile technique procedures.
Experience working with individuals with disabilities or developmental challenges such as behavioral health needs or pediatric patients including neonatal ventilator management.
Certifications in basic life support (BLS), CPR, first aid, catheterization techniques, and infection control policies are required.
