Full Job Description

SUMMARY:

Under direct supervision from the Director, contributes to the efficient daily operation of the HealthWellness department by performing a variety of community outreach duties in support of clients and in accordance with the established policies and procedures of Lea County. Maintains professionalism in all interactions with internal and external customers.

ESSENTIAL DUTIES:

Monitors treatment and educational referrals; aids clients in accessing services with outside agencies and resources to improve educational, health, occupational, relational, emotional and behavioral goals.
Assesses client needs and case progress; compiles case reports to ensure inclusion of relevant information to link available services with the client's overall well-being.
Develops and maintains strong working relationships with external agencies; conducts site visits with external agencies as needed.
Documents, files, and maintains case histories; prepares reports as required.
Serves as a liaison for treatment and government organizations, schools, courts, and other essential services.
Develops and maintains a comprehensive and current list of local, state, and federal resources; maintains protocols for working with each agency; updates departmental staff updated regarding resources.
Completes required training and maintains knowledge in the areas of domestic violence advocacy, behavioral health resources, best practices for linking services, and other needed knowledge and skills.
Facilitates educational groups, develops and evaluates pre- and post-tests, and maintains class rosters.
Develops and preserves treatment orientation process and referral process from other departments and agencies.
Assists Director and treatment staff with case file preparation.
Cross-trains with other departmental staff to support team goals, activities, and workloads.
Appears in court or at prevention events as required.
Participates in community events.
Represents department and attends meetings as required.
Screens incoming calls and correspondence; exercises judgment and responds accordingly.
Responds to questions and requests for information in a professional and timely manner.
Establishes and maintains an effective system of communication within the department and builds and fosters effective working relationships with others.
Complies with all property and department policies and procedures.
Maintains a professional, organizational, and community reputation.
Maintains strict confidentiality of all information processed through the department including records, reports, documents, conversations, etc.
Contributes to a team effort and accomplishes related results as required.
Performs other duties as assigned.

The duties of this position are not limited to those set forth above. The incumbent will perform such functions, duties and assignments as directed by supervisor and consistent with ability, background and expertise.

MINIMUM QUALIFICATIONS:

Associate's degree in Social Services or related field.
Two (2) years' experience in public relations, community education, or related environment.
Must achieve and maintain Professional Recovery Coach certification.
A combination of relevant education and related work experience may be considered.
Valid driver's license is required when operating a vehicle for work-related purposes.
Must successfully complete pre-employment requirements including a background check and drug/alcohol screening.

Knowledge, Skills, and Abilities:

Knowledge of:

Goals, objectives, functions, policies, and procedures of Lea County.
Department organization, functions, objectives, policies and procedures.
Public relations.
Program development.
Records management practices.
Modern office functions, procedures, and equipment.

Skill in:

Operating various word-processing, spreadsheets, database software programs, and industry-related applications in a Windows environment.
Reading, reviewing, preparing, and analyzing operational reports, contracts, minutes, resolutions, ordinances, and other business-related documents.
Analyzing problems, projecting consequences, identifying solutions, and implementing recommendations.
Making effective decisions in urgent situations.
Correct English usage, grammar, spelling, and punctuation.
Exceptional customer service.

Ability to:

Perform essential duties and adapt to working conditions.
Maintain strict confidentiality.
Communicate efficiently and effectively both verbally and in writing, individually and in meetings.
Create engaging and informative presentations and educational materials.

PHYSICALEMOTIONAL DEMANDS:

While performing the duties of this job, the employee regularly is required to sit; stand; walk; reach with hands or arms; stoop, kneel, crouch, or crawl; use hands to manipulate, touch, or grasp; use a computer and cellphone/telephone; talk or hear; and lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

WORKING CONDITIONS:

Work is generally performed in an office and outdoor setting with a moderate noise level. The employee may be occasionally exposed to hostile, violent, and/or offensive individuals and excessive noise. Evening and/or weekend work is regularly required. Tight time constraints and multiple demands are common. Travel is required for training, meetings, conferences, presentations, and other events. Incumbent is subject to call out at any time.
