Case Coordinator

Job Summary: We're looking for a compassionate, resourceful, and driven Case Coordinator to join our team full-time. In this role, you'll be a lifeline for immigrant individuals and families – stepping in during moments of crisis, connecting people to critical services, and walking alongside them as they build stability and independence. You'll work across our three locations in Mount Kisco, Ossining and Yonkers, collaborating with a dedicated team of social service professionals and community partners. This position will also collaborate closely with our team of immigration attorneys and paralegals to help ensure clients receive holistic support that strengthens both family stability and immigration legal outcomes. You will report to the Senior Manager of Client Support Services. This is a role for someone who thrives in a dynamic environment, leads with empathy, and believes deeply in the dignity and strength of every client they serve.
Responsibilities:

● Meet clients where they are – bringing a culturally sensitive, creative, and fl exible approach to working with individuals and families from diverse backgrounds.
● Work regularly from the office, meet clients in person in our Mount Kisco, Ossining and Yonkers offi ce (two full workdays (9am-5pm) in our Mount Kisco location and at least one additional workday per week to meet with clients in person our in Ossining and/or Yonkers offi ce), in addition to participating in internal and external meetings with staff and community partners.
● Partner with local non profi t and partner agencies to provide community-based client support (including but not limited to picking up clothing donations from The Sharing Shelf and coordinating pick up with clients, accompanying clients to Furniture Sharehouse or Teen Boutique when needed).
● Manage an active client caseload with careful documentation, detailed case notes, and consistent follow-up to ensure no one falls through the cracks.
● Show up for clients in crisis – helping them access a wide range of services while building their confi dence and autonomy for the long term.
● Select and apply appropriate intervention strategies that support client growth and align with our mission of integration and empowerment.
● Build and maintain strong relationships with community partners (i.e. Open Door, Northern Westchester Hospital, Mental Health Association, Catholic Charities, among others) to ensure seamless, direct referrals for clients with specifi c needs.
● Conduct thorough client assessments and re-assessments, and close cases thoughtfully when goals have been met.
● Partner closely with our immigration legal team, providing case management support and regular status updates for clients referred for case management.
● Participate in team meetings, training sessions, and professional development workshops to continuously grow in your role (both in person and remotely).
● Escalate time-sensitive client situations to your manager promptly and proactively.
● Assist with additional program activities as opportunities arise.
You’re a great fi t if you:
● Are English-Spanish bilingual or multilingual (strongly encouraged and refl ects the diversity of the communities we serve).
● Have an undergraduate degree in Social Work, Case Work, Counseling, Psychology or related fi eld, LMSW or LCSW welcomed.
● Have at least two years of previous experience in a similar position. ● Are adept at multitasking and ability to manage crises.
● Have strong clerical/administrative skills including personal computer skills (i.e., Microsoft Offi ce, Google Drive, Salesforce a plus).
● Have excellent communication (written and verbal) and organizational skills.
● Are willing and able to support other programs and/or special events, including community outreach eff orts in the evening or on weekends.
● Have strong interpersonal and communication skills and the ability to work eff ectively with a wide range of constituencies in a diverse community.
● Are able to be on-site two full workdays (9am-5pm) in our Mount Kisco location and on at least one additional work day per week to meet with clients in person in our Ossining and/or Yonkers offi ce.
● Have fl exibility to adjust work locations and schedules based on program needs, client needs, internal and external meetings and organizational priorities.

You have seen on the news how immigrants seeking asylum in the United States have been bussed to New York and other cities without adequate support or resources. As a bilingual Case Coordinator at Neighbors Link, you will work with families from all over the world who hope to make America their home. You’ll provide a deep level of support to a caseload of families as they move from temporary shelters to permanent homes and become integrated into lower Hudson Valley area communities. Several full- and part-time positions are available. These are two-year positions with full benefits at Neighbors Link, a high-performing nonprofit organization that supports immigrants with a wide array of programs. After the two-year period, opportunities may be available to transition to a different role within the organization.

You care deeply about recent migrants and understand how complicated and traumatic their experience might be. You work with individual members of a family according to what each one needs. You are grateful for an opportunity to build a trusting relationship with families who need a range of support: permanent housing, work, schooling for children, food, clothing, furniture, and more. Despite obstacles, you know—or can quickly learn—how to get things done to meet families’ basic needs. You can also juggle about 25 families’ needs at once and will continue working with each for at least one year.
Responsibilities
- Reflect the values and philosophy of Neighbors Link by creating a caring and urturing environment based on mutual respect. Work with individuals and families with a culturally sensitive approach that allows room for creativity and an opportunity to “meet clients where they are.”
- Support clients in crisis, assist individuals and families in accessing a wide variety of services. Choose an appropriate intervention technique to empower clients and cultivate individual empowerment.
- Manage a caseload of 25 families and document detailed case notes to ensure appropriate follow-up and record keeping.
- Establish service linkages, make necessary referrals, and monitor service delivery to ensure that clients are being appropriately supported. Collaborate and strengthen relationships with community partners (for example, Open Door, Northern Westchester Hospital, Mental Health Association, Catholic Charities) in order to make direct referrals for clients with specific needs.
- Conduct assessments and re-assessments of clients, including closing cases as appropriate.
- Participate in team meetings, assigned trainings, and educational workshops.
- Consult with and report to Case Manager on new clients who may need immediate support based on needs/risk. Provide regular updates to the legal team and other departments as appropriate.
- Interface with the Volunteer Manager, and volunteers as needed.
- Assist with other program activities as assigned.
Requirements
- Fluency in English and Spanish required. Working knowledge of other languages is welcome (for example, French, Russian, Arabic, or African languages) but not required.
- Undergraduate Degree in Social Work, Case Work, Counseling, Psychology or related field At least two years of previous experience in a similar position preferred.
- Your own transportation for travel mostly in Westchester but also in lower Hudson Valley.
- Home base will be at Neighbors Link in Mount Kisco, NY; willing to work from flexible office space and on the road.
- Work independently AND provide detailed accountability to your supervisor.
- Patience in working with families who have been unstable for an extended period of time in a new country.
- Knowledge of social service systems (in any location) and persistence in getting services for clients.
