Full Job Description

Weekend On-Call Care Coordinators – Two Positions Available

QHM Homecare Caregiver Recruitment is seeking two reliable and dedicated Weekend On-Call Care Coordinators to assist caregivers, clients, and families outside regular office hours.

Location: Plainfield, CT

Schedule:

Saturday: Work in the Plainfield office
Sunday: Work from home
Additional hours available: Monday–Friday, 4:00 PM–8:00 PM

Immediate start — full training provided!

Responsibilities

Respond to calls from caregivers, clients, and clients’ families.
Find coverage when caregivers call out or shifts become available.
Contact caregivers and offer available client shifts.
Respond to urgent scheduling questions and concerns.
Take detailed notes regarding calls, schedule changes, and important updates.
Communicate weekend and after-hours updates to the Care Coordination Team.
Learn different areas of the company to provide effective off-hours support.
Escalate serious concerns to the appropriate manager when necessary.

Minimum Qualifications

Dependable and willing to learn.
Friendly and professional telephone manner.
Basic computer skills and willingness to learn multiple systems.
Organized and able to take clear, detailed notes.
Comfortable communicating with caregivers, clients, and families.
Able to remain calm and professional during unexpected situations.
Able to work independently and as part of a team.
Reliable transportation and the ability to pass a background check.
A quiet and dependable work environment for Sundays.

No home-care coordination experience is required. We provide comprehensive training and ongoing support to help the right candidates learn the position and succeed.
