The Anacortes Family Center (AFC) is seeking a Full-Time, Non-Exempt, Program Intake Coordinator to assist with vulnerable, homeless or unstably housed community members, connecting them with services that are internal to the Anacortes Family Center. (Participants needing external services would connect through this position's partner, our Community Resource Coordinator). AFC provides emergency support through outreach, motel vouchers, 60-90-day emergency shelter, 12-24-month transitional housing, affordable housing, case management and life skills. This position specifically works to fill our Emergency Shelter - so works closely with families with children, and single women, in homelessness - connecting them to resources while completing all of the necessary steps to work through our waitlist and potentially admission to our emergency shelter.
Schedule: Monday-Friday; 9:00AM-5:30PM (with 30 min lunch) or after training, some flexibility will exist to create your 8-hour/day 40 hour work week. Limited on-call work is necessary

Compensation:
· $58,000/year - $61,000 Salary (Non-Exempt); Pay is evaluated each calendar year and each position is reviewed for merit and cost of living increases. Depending on the organization's ability, year-end bonuses are offered, when able.

Job Description:
Anacortes Family Center's Program Intake Coordinator (PIC) assists those experiencing homelessness, those vulnerable to homelessness and/or those in crisis, The goal of the PIC is to provide direct support to our community’s most vulnerable by supporting them in the process to apply to AFC’s Emergency Family (or single female) Shelter (families are defined as any parent, of any gender, with child(ren) in the household). The PIC continues to follow-up with the applicant each week generally via regular phone calls and occasional on-site meetings, and in the process, aids by helping them access social services (DSHS, employment programs, foodbanks, etc.), submitting housing applications (via housing authority, shelters, transitional programs, etc.), connecting to mental health services, healthcare, chemical dependency services, etc. as well as referring the household to services within the Anacortes Family Center’s programs (e.g. motel vouchers, emergency shelter, counseling, transportation support, education support for kids, etc.). The PIC is responsible for working one-on-one with each household to identify their obstacles and to develop weekly action plans, with the aim to prepare them for the best success once entering our 60-90 day emergency shelter. The PIC serves as a link to community resources, service providers, and those in need.
High Level Functions:
· Work with AFC’s Program Team to support clients in crisis
· Manage AFC’s Emergency Shelter waitlist: This requires a substantial amount of time spent on the phone checking in with clients to ensure updated information. In person meetings in our office will also occur on a semi-frequent basis.
· Work with applicants to assess their fit for AFC’s emergency shelter
· Complete initial assessment paperwork for the County’s Coordinated Entry System
o Identify family’s needs that will impact their program readiness and factors that will impact them while in Case Management in the program.
· Staff potential new clients with AFC’s program team (Program Director, Asst. Director/Emergency Shelter Case Manager, Program Coordinator, Community Resource Coordinator, Associate Youth Director, etc.)
· Assess clients already enrolled in Motel Voucher Program (with the point for the Motel Voucher Program – the Community Resource Coordinator) and determine when the client should be transferred from the CRM caseload to the PIC case load, or vice versa
Pre-Intake Case Management & Intake Process
· Schedule/conduct intakes:
o The PIC and each client/household must be in contact no less than three times (as noted in our client relationship software, Salesforce) to review all program expectations and to update the client profile with any pertinent changes that may impact the client’s eligibility/case.
o Intakes generally take 1.5-3 hours and require a high level of detail orientation and ability to keep clients on track.
o Prior to entry, all AFC clients must successfully pass a urinalysis to check for unauthorized drug use. This occurs during the intake process, and is done with the PIC and one other staff member for integrity/safety.
· Provide one-on-one supportive services. Meet with community members in-need on a regular basis and refer, coordinate and connect to available services
· Collaborate and network with community members, local agencies (Anacortes Police, Island Hospital, Parks and Recreation, etc.) and others (churches, civic clubs, other providers) to assess and address needs and identify those in need of support
· Engage in our County’s network of Emergency Shelter Providers, and the work group that is charged with our Coordinated Entry system. These meetings may occur monthly and may happen in Mount Vernon.
· The PIC acts as a resource for local law enforcement and City officials – supporting them in understanding available services (providing technical insight and sometimes technical support), and helping to find ways to bridge gaps in available resources.
o The PIC helps to bring new resources into the AFC network of services. That can include creating space for providers to meet with clients on-site, helping other providers with securing resources that would add resources outside of AFC’s direct mission, etc.
· The PIC is the co-coordinator for our Fidalgo Island Point-in-Time Count: The Point-in-Time Count is a national day of identifying homeless individuals to assess community need. AFC is the lead agency for Fidalgo Island (Volunteers of America oversees the Point in Time for the rest of Skagit County). Supporting Point in Time includes attending planning meetings, creating “magnet” events to bring people in to get counted, working with other providers (food banks, the School District, etc.), and working to train and coordinate volunteers.
· Maintain thorough confidential case notes and files for all clients in a timely manner
· Input and manage data in systems, including HMIS, SalesForce, and others
· De-escalate crisis situations with individual clients and ensure client safety and stability through safety plans, collaboration with the Crisis Prevention Intervention Team, welfare checks, follow-up meetings and additional coordination
· Provide accountability and enforce program expectations to encourage client progression and stability
· Attend and participate in staff meetings (each Monday); The Program Team meets as a group each Thursday to staff clients and review upcoming openings/intakes/etc.
· Assist with program development and policy and procedures
· Ensure compliance with all relevant laws, including fair housing laws, confidentiality and privacy requirements
· Respond to after-hours emergencies, and take part in the rotation of motel voucher coverage on holidays, as needed
Skills & Abilities Required:
- At least 2 years of experience working in the field of case management, human services, crisis prevention and/or housing
- Degree in Human Services, Social Work, Counseling or similar field, or equivalent relevant work experience
- Excellent verbal & written communication skills. MUST be comfortable with spending a majority of the day on phone calls to prospective clients.
- Excellent computer skills- email, data entry, MS office, and Salesforce (or similar program) experience a plus
- Excellent interpersonal skills and ability to positively interact with individuals of diverse backgrounds
- Ability to work with a team and collaborate with other staff
- Excellent time management skills and ability to work with frequent interruption and often in crisis situations
- High level of professionalism
- Ability to maintain accurate and timely records
- Ability to address conflict and manage crisis situations
Desired Qualifications:
- Background in chemical dependency is useful
- Bilingual Spanish/English is a plus
- Experience in working with people in crisis
- Familiarity with Fair Housing Laws and community resources
