Job Summary

We are seeking a dedicated and knowledgeable Nurse Assessor to join our healthcare team. In this vital role, you will evaluate patient conditions, review medical records, and determine appropriate care plans to ensure optimal patient outcomes. Your expertise will support long-term care facilities, hospitals, and healthcare organizations by providing comprehensive assessments that adhere to regulatory standards. This position offers an exciting opportunity to utilize your clinical skills, leadership abilities, and regulatory knowledge to make a meaningful difference in patient care and healthcare quality. Duties

Conduct thorough assessments of patients’ health status, including physical examinations and review of medical histories
Evaluate medical documentation and records to determine eligibility for services or benefits in compliance with healthcare regulations
Collaborate with multidisciplinary teams to develop individualized care plans tailored to each patient’s needs
Utilize electronic health records (EHR) systems to document findings accurately and efficiently
Ensure all assessments meet federal and state healthcare regulations, including HIPAA privacy standards and long-term care regulations
Provide guidance on medical terminology and physiology concepts during evaluations and documentation processes
Lead or participate in quality improvement initiatives aimed at enhancing patient care standards and regulatory compliance

Skills

Proficiency in Microsoft Office applications for documentation, reporting, and data analysis purposes
Strong management skills to oversee clinical assessments and supervise administrative staff when needed
Extensive experience in patient care within hospital or long-term care settings with acute care expertise
Deep knowledge of physiology, anatomy, medical terminology, and medical records management
Familiarity with Electronic Health Records (EHR) or Electronic Medical Records (EMR) systems for efficient data handling
Understanding of HIPAA regulations and long-term care facility requirements to ensure confidentiality and compliance
Ability to analyze medical data critically to support decision-making processes
Knowledge of state healthcare regulations governing nursing assessments and long-term care practices
Proven nursing leadership experience with the ability to provide administrative clinical supervision
Strong communication skills for clear documentation and effective collaboration with healthcare teams
