Full Job Description

Position Summary

We are seeking an experienced and compassionate Registered Dietitian Nutritionist (RDN) with an active New York State Certified Dietitian-Nutritionist (CDN) credential to join our team. The Dietitian will conduct clinical nutrition assessments, develop individualized nutrition care plans, review medically tailored menus, approve meal recommendations, and sign clinical documentation to ensure compliance with organizational, regulatory, and evidence-based standards.

Key Responsibilities

Conduct comprehensive nutritional assessments and evaluations.
Develop, implement, and monitor personalized clinically appropriate Meals .
Provide one-on-one dietary counseling.
Maintain accurate, HIPAA-compliant electronic health records and documentation.
Collaborate with care managers, providers, and interdisciplinary team members regarding patient nutrition needs.
Participate in quality assurance activities and menu reviews.
Stay current on clinical nutrition guidelines and maintain all required continuing education.

Qualifications & Skills

Education: Bachelor's or Master's degree in Dietetics, Nutrition, or a related field.
Active RDN and New York CDN credentials (required).
Strong communication, active listening, and counseling skills.
Experience working with Medicaid populations, managed care organizations, or medically tailored meal programs is preferred.
Ability to work independently and manage a diverse caseload.
Preferred 1-2 years experience.
