Pine Ridge Terrace, an Independent Living, Assisted Living, and Memory Care senior living community, is seeking a compassionate and highly organized Resident Care Coordinator to support the delivery of exceptional resident care services. The ideal candidate will have senior living experience, strong leadership and communication skills, and a passion for enhancing the health, safety, and well-being of residents. This role partners closely with the Health Services Director to coordinate care services, support staff training and scheduling, monitor clinical documentation and medication management systems, and ensure compliance with applicable regulations and community standards.

Position Summary:
Assist Health Services Director to assure health services of assigned residents are addressed, as well as other services issues that have a potential impact upon the health of residents.
Essential Job Duties & Responsibilities:

Perform health services duties, including but not limited to:
- Utilize OnShift to assist Health Services Director with the completion monthly work schedule for Med Techs and Resident Assistants.

Assist Health Services Director to secure adequate staffing to meet resident needs due to absences.
- Assist Health Services Director to ensure Resident Assistants provide care and services according to service plan and daily assignments.

Assist Health Services Director to assure Medication Techs and Resident Assistants are educated and trained using available Resource Manuals, Care Academy and on the floor mentoring.

Assist Health Services Director with the completion of orientation skills checklists on all new hires.

Assist Health Services Director conduct continuing education and training for Medication Techs and Resident Assistants as necessary.

Assist Health Services Director with completing ongoing assessments of residents 30 days after moving in, then every 6 months or upon change of condition.

Assisted Health Services Director with the monitoring of physician orders and correct implementation.

Assist Health Services Director with monitoring laboratory and other test results.

Document all activities in Yardi electronic health record.

Monitor MAR:
- Assist Health Services Director to assure medications and treatments are administered as ordered, including by reviewing missed medications report.

Assist Health Services Director to assure parameters for administration of PRN medications and treatments are entered MAR.

Assist Health Services Director to assure appropriate diagnosis for medications and treatments are entered on MAR.

Assist Health Services Director to address issues regarding unavailable medications or treatment supplies
- Assist Health Services Director to establish weight regimens, assure weights are entered on MAR or other appropriate location, and address all significant weight changes.

Assist Health Services Director to establish vital sign regimens and related notification parameters; assure entered on MAR and adhered to.

Assist Health Services Director with monitoring of alert charting, especially to assure appropriate employee response to issues.

Assist Health Services Director with monitoring of skin problems using Current Skin Problems form; address identified skin problems.

Assist Health Services Director with monitoring of inter-shift health services report.

Assist Health Services Director with monitoring of Communication Log.

Assist Health Services Director with monitoring and addressing consulting pharmacy reports.

Assist Health Services Director to advise health services employees concerning resident health issues, by telephone if not in community (carries cellular telephone).

Assist Health Services Director with effectively utilize and coordinate services with home health, hospice, and other agencies.

Assist Health Services Director to assure physician orders are reviewed, sent to physician, and returned with physician signatures quarterly.

Assist Health Services Director to investigate controlled substance count discrepancies.

Assist Health Services Director to assure controlled substances are destroyed as necessary per policy and state or provincial regulations.

Assist Health Services Director with monitoring to assure Assisted Living Daily Assignments duties are performed.

Perform quality assurance audits as directed.

Effectively communicate with residents, their family members, and other concerned parties.

Effectively communicate with physicians and other health care professionals involved with services to residents.

Assure compliance with all federal and state or provincial regulations pertaining to health services.

Assist Health Services Director with maintaining Daily Assignment system.

Assure physician orders, laboratory reports, and other data is scanned and filed in resident chart.

Maintain Medication Room files per policy

Maintain Tracking Reports binder per policy.

Maintain Pending Fax Communication System per policy.

Assist Health Services Director to coordinate annual flu vaccine clinic.

Assist health services employees with duties as necessary.

Maintain resident, employee, and community information in strict confidence (with community management or other employees).

Strong Leadership Skills, including strong interpersonal and communication skills.

Demonstrate ability to communicate effectively, both verbally and in writing.

Meet state or provincial health related requirements.

Maintain CPR and first aid certification.
-
Maintain any certifications as required by state or provincial regulations.

Experience in RCFE Senior Living and dementia care preferred.
