CNA/Personal Care Attendant (Home Health)
FT or PT available, shift is 11am - 9pm. CNA certification is required.
Region VII Area Agency on Aging/Sunrise PACE is accepting applications for a highly motivated CNA/Personal Care Attendant (Home Health Care) for Sunrise PACE. Purpose of the position:
Under the direction and supervision of the Home Care Coordinator, the CNA/Personal Care Attendant provides personal and home care services to Sunrise PACE participants in their homes. Provides physical assistance and attendant care to enrollees across care settings (home, transportation, on occasion day center). Provides assistance with Instrumental Activities of Daily Living (IADLs). Provides assistance with nursing care, restorative therapies, activities and meals across care settings. Demonstrates the knowledge and skills necessary to participate in assessing, planning care for and providing service to frail elder enrollees according to assigned responsibilities and (PACE) Program standards. This CNA/PCA position will primarily be for home health care of Sunrise PACE participants.
Responsibilities & Duties (this is not an all inclusive list)
- Geriatric Caregiving
- Provides physical assistance and attendant care to participants across care settings (home, center, transportation)
- Provides basic personal care assistance and/or supervision and assistance with any other Activities of Daily Living (ADLs)
- Assists w/bathing in tub and/or shower
- Assists w/transferring on/off bedpan, commode, toilet
- Provides care of teeth & mouth
- Assists w/grooming, eating, dressing as needed
- Performs and/or assists w/laundry, homemaking, meal prep, light cleaning & other household duties such as chores service
- Provides assistance w/nursing care restorative therapies, activities & mails across care settings (such as:)
- Assist w/ambulation, transfer & range of motion needs as determined by physical and/or occupational therapist
- Provides skin care w/intact skin
- Takes temperature (oral and rectal), pulse & respiration when advised to by RN
- Measure intake & output as ordered
- May assist in day center to include
- Set up and operate recreational, health care & other equipment
- Lead games, singing, other activities
- Assist in keeping Center in clean orderly condition, including showers, bathrooms, kitchen & activities area
- Potentially participant in development & revision of participant's plan of care as member of interdisciplinary team
- Reports noted changes in participant's abilities to self-care to supervisor and the interdisciplinary team
- Documents all services clearly and completely
- Maintain standard records of work performed
Requirements for this position
- Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant care
- Pass a criminal background check which will verify that the candidate is: not excluded from participant in Medicare & Medicaid programs
- High school diploma or equivalent
- CNA certification required
- 1 year experience with the frail or elderly population or, in the absence of such experience, to receive appropriate training from the Sunrise PACE on working w/frail or elderly population upon hire
- Ability to perform housekeeping duties
- Initiative and resourcefulness
- Use of discretion, tact, patience and good judgement. Empathetic and compassionate attitude in caring for the elderly
- Ability to understand and carry out non-technical verbal and written directions, record messages and keep records.
- Ability to get along well with others and gain the cooperation of plan participants
- Able to handle difficult situations in a professional manner
- Routine work involving the personal care and program activities of participants.
- Proven record of excellent attendance
- Ability to maintain flexibility in schedule and respond to unexpected emergencies and changes in workload
- Ability to spend long periods of time on feet and do heavy lifting on occasion
- Licensed driver with reliable transportation
