Full Job Description

Sunrise Services, INC. is seeking a Case Manager for our Behavioral Health program!

About the Program:

Our office is known for its strong commitment to taking care of those in need in our local community. We provide persons in need of mental health support with wraparound services which include counseling, therapy, EMDR, medication management and employment services. Our client population includes chronically homeless individuals and/or those with persistent and serve mental illness.

Our staff is comprised of a tightly knit team of local therapists, case managers and psychiatric prescribers. Candidates can expect to provide mental health counseling and case management.

At Sunrise Services, we offer an initial pay scale for our fully licensed, associate licensed and case manager position. This rate reflects not only direct client hours but also includes client no shows and client cancellations. This gives our providers peace of mind, guaranteeing steady income in a field that can be inconsistent at times.

Duties & Responsibilities:

Provide ongoing coordination and collaboration of case management services
Provide treatment planning, resource identification and development, and advocacy
Provide service monitoring and follow-up
Physical health care needs: Refer client to physician to address physical care needs, assist client in attending appointments. Assist the client to inform the prescriber for psychiatric med management services of their physical health care plan.

Requirements: (Any Equivalent Combination of Knowledge, Skills, Abilities, Education, and Experience)

Education: Bachelor's in Psychology, Social Services, or related field.
Licensure: Must have a Registered AAC or the ability to obtain one.
Experience: Supervised practical experience in a clinical or rehabilitation (recovery) setting with persons with severe and persistent mental illnesses.

Schedule:

M – F, 8:00am – 5pm

Compensation:

$22.05 - $23.54/hour
Wages are determined by applicable industry experience, education, and certifications/licensure.

Paid vacation - Earned at .04 per hour worked for hourly employees, and 6.67 per month for salary (equates to about 2 weeks per year for full-time employees)
Sick Leave - 1 hour per 40 hours worked (equates to about 52 hours for full-time)

Paid ‘Floating’ Holiday to be used on a day of choice
