Full Job Description

Pain and Palliative Associates is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing practice serving the Prescott area. The provider will initially focus on rounding at three skilled nursing facilities while helping expand palliative care services throughout the community.

This position offers a flexible schedule, with required rounding twice weekly at each of three skilled nursing facilities. As the practice grows, the role may expand to include inpatient palliative care consultations at the local hospital and outpatient follow-up for patients discharged from the hospital or skilled nursing setting.

This is a 1099 independent contractor opportunity with excellent earning potential for a motivated provider who values autonomy, flexibility, continuity of care, and the opportunity to help build a growing palliative care program.

Position Highlights

Meaningful role in the continued growth of the Prescott-area practice

Responsibilities

Perform comprehensive evaluations and follow-up visits for patients residing in skilled nursing facilities.
Provide symptom management and supportive care for patients with serious, chronic, or life-limiting illnesses.
Manage complex symptoms, including pain, dyspnea, nausea, anxiety, delirium, fatigue, constipation, and other sources of distress.
Facilitate goals-of-care discussions and advance care planning, including code-status conversations and completion of advance directives when appropriate.
Educate patients and families regarding disease progression, treatment options, and hospice eligibility when appropriate.
Collaborate closely with facility physicians, nursing staff, specialists, primary care providers, case management, hospice agencies, and other members of the interdisciplinary care team.
Coordinate transitions of care among skilled nursing facilities, hospitals, outpatient settings, hospice, and home.
Complete timely and accurate documentation within the electronic medical record.
Maintain strong relationships with facility staff and referring providers while delivering compassionate, patient-centered care.
Assist with the development and growth of inpatient and outpatient palliative care services in the Prescott area.
As services expand, perform inpatient palliative care consultations and outpatient follow-up visits for patients transitioning from the hospital or skilled nursing setting.

Qualifications

Current Arizona licensure as a Nurse Practitioner or Physician Assistant
Current national board certification
Active DEA registration
Current malpractice insurance
Previous advanced practice experience preferred
Experience in skilled nursing, palliative care, hospice, internal medicine, hospital medicine, geriatrics, oncology, critical care, or pain management strongly preferred
Pain management or palliative care board certification preferred
Providers who are not currently certified will be expected to obtain pain management or palliative care board certification within 12 months of beginning the contract engagement
Ability to work independently, manage a flexible rounding schedule, and collaborate effectively with facility-based interdisciplinary teams

Training and Compensation

The full compensation rate is $35 per completed patient encounter as a 1099 independent contractor.

A structured training and onboarding period will be provided. Depending on the providers experience, demonstrated competency, and ability to work independently, this period may range from 30 to 90 days. During training, compensation will be $30 per completed patient encounter, which is approximately 85% of the full encounter rate. Following successful completion of training and confirmation that the provider can fulfill the role independently, compensation will increase to $35 per completed patient encounter.

Providers who complete an approved pain management or palliative care board certification within the first 12 months of their contract engagement will be eligible for consideration for an additional compensation increase.

Estimated earning potential is approximately $120,000–$240,000 or more annually, depending on productivity, patient census, facility volume, scheduling, and the growth of hospital and outpatient services. Established facility relationships provide immediate patient volume with continued opportunities for professional and financial growth. Earnings are estimates and are not guaranteed.
