If you've spent your career working inside a hospital, SNF, outpatient clinic, or rehabilitation facility, you're probably accustomed to someone else controlling much of your day.
Your first patient is waiting. Your next patient is already arriving. Productivity expectations are constantly in the background.
Home health offers a different kind of professional independence.
You'll manage a territory-based caseload, travel directly to your patients, and use your clinical judgment to determine what each individual needs to safely function at home.
You're still accountable for excellent outcomes.
But you're trusted to practice like the experienced clinician you are.
Your Clinical Skills Have a Different Impact at Home
When you evaluate a patient in their home, you don't have to recreate the challenges they're facing.
They're right in front of you.
The stairs they need to climb.
The hallway they're struggling to navigate.
The front steps preventing them from leaving the house.
The chair they can no longer stand from independently.
You'll develop treatment plans around the patient's actual environment, actual goals, and actual barriers to independence.
That's where Physical Therapy can have an immediate impact.
What You'll Do
- Provide in-home Physical Therapy evaluations and treatment visits.
- Complete admissions and start-of-care assessments.
- Develop individualized treatment plans based on patient needs and goals.
- Address strength, balance, gait, mobility, transfers, and functional independence.
- Identify fall risks and barriers to safely remaining at home.
- Educate patients, families, and caregivers.
- Supervise and collaborate with Physical Therapist Assistants.
- Coordinate care with nurses, OTs, SLPs, physicians, and other interdisciplinary team members.
- Monitor progress and modify plans of care as appropriate.
- Complete timely clinical documentation, including OASIS when applicable.
Who We're Looking For
We're interested in Physical Therapists coming from Home Health, Outpatient Therapy, Acute Care, Inpatient Rehabilitation, Skilled Nursing, Transitional Care, Orthopedics, Senior Living, and other rehabilitation settings.
Previous home health and OASIS experience are preferred, but we're also interested in strong clinicians who are ready to transition their skills into the field.
Qualifications
- Current Minnesota Physical Therapist license.
- Strong clinical assessment and decision-making skills.
- Ability to independently manage a territory and caseload.
- Valid driver's license and reliable transportation.
- Excellent patient and caregiver communication skills.
- Home health experience preferred.
- OASIS experience preferred.
Work Hard During the Week. Keep the Weekend for Everything Else.
We're not selling a PT position where "flexibility" means you're still checking whether you're scheduled Saturday.
This is Monday through Friday.
