Duty: Clinical Skills 80%
Under the direction of a Registered Nurse is responsible to: administer nursing care (both psychiatric and medical) according to the plan of care, monitors and facilitates the provision of a therapeutic, safe milieu, and manage the milieu in the absence of HCT II or HCT III. Administer meds and treatments according to licensing and CMHIP standards for example oral, injection and topical. Follows all patient identification process for med administrations, treatments, etc. Aware of patient allergies, sensitivities of food/drug interactions, drug/drug interactions. Monitors all aspects of patient’s care needs to include nutritional, dysphagia and allergies.
Responsible to respond to safety and security issues/needs. Utilizes therapeutic interventions and specialized training to control and manage crisis situations and management of assaultive and disruptive behaviors. Utilizes proper seclusion and restraint procedures following hospital/nursing department policy and procedure. After a seclusion and restraint episode, ensures the restraints are all gathered, counted and placed back into the restraint bag. Notes any restraints that need replaced on the RIPP check off sheet and alerts the correct staff member to replace.
Knows and follows all nursing procedures, computerized trainings/nursing groups/competencies/addenda/patient teaching/staff training and competency. Understands, promotes, and participates in Trauma Informed Care initiatives. Is a resource and teaches/orients new/pulled/pool staff related to milieu management/safety basics. Understands and maintains staff/patient boundaries. Utilizes clean, aseptic technique and sterile technique when required. Follows hospital policy and monitors own use of time
Duty: Accountability 10%
Knows, utilizes and follows CMHIP standards and guidelines: Treatment Plans, Infection Control Manual, and Rainbow book and Emergency Guides, Responsible to maintain own licensure and not allow it to lapse. Follows and maintains infection control standards and regulations. Adheres to CDHS Code of Conduct and CMHIP Code of Ethics policies.
Follows and complies with all nursing competencies related to the HCT I. Is aware and utilizes the Nursing chain of command. Completes medication variances when necessary. Provide safe/secure transport of patients to yard, gym, visits, administrative conference room, staffing/dispo reviews, court/community reintegration sites. Documents towards Nursing Department standards. Knows how to access the nursing drive and is expected to utilize it.
Responsible to ensure educational profile is current and up to date. Participates in the QA/PI process at the unit level as directed by the Lead Nurse. Attends weekly nursing rounds or read/review and understand information in the weekly NEC minutes to be aware of nursing direction, new information, changes in policy/procedure/instructions etc. Reports any claims of any type of harassment/work place violence, alerts public safety/charge RN/Lead Nurse/and nursing appointing authority.
Duty: Communication/Interpersonal Skills 5%
Effectively communicates by actively listening and sharing relevant information with co-workers, supervisor(s) and customers so as to anticipate problems and ensure the effectiveness of the department. Adapts communication methods to respond to different audiences to promote a positive environment. Is appropriate in all communications with co-workers and patients. Receives cross shift report from the off going RN and is aware of all 1:1, SP II, medical and psych issues for each patient. Addressing patient complaints/complaints of family, and resolving patient grievances at the unit level. Reports abuse allegations immediately to the charge RN/Lead Nurse/nursing appointing authority and refers to public safety. Alerts the RN to all exceptions, changes in patient status to include suicidal/homicidal ideation, need for medication or RN intervention. Completes and routes paperwork to appropriate area/person (to include incident reports and injury on the job,). Reports and corrects any hazardous findings. Other duties as assigned.
