JOB TITLE: ECM Case Manager
Full-Time: $22.00-$28.00/hr DOE

PURPOSE OF THE POSITION:
The Enhanced Care Management (ECM) Case Manager provides comprehensive, trauma-informed case management services to high-risk Medi-Cal beneficiaries. Working as part of an interdisciplinary care team, the Case Manager coordinates access to healthcare, housing, behavioral health services, and other critical supports, addressing social determinants of health and ensuring the successful implementation of client-centered care plans.
Reports to ECM Program Manager.
QUALIFICATIONS:
MINIMUM:
- High School diploma
- 1+ years of experience in case management, social services, behavioral health, or care coordination.
PREFERRED:
- Bachelor’s degree in Social Work, Psychology, Public Health, or a related field.
- Knowledge of CalAIM, Enhanced Care Management (ECM), and Medi-Cal service systems.
- Experience working with high-risk populations, including individuals with mental illness, substance use disorders (SUD), chronic health conditions, and housing insecurity.
- Experience with managed care coordination, and community resource navigation.
OTHER REQUIREMENTS:
- Valid unrestricted driver’s license, reliable transportation, and vehicle insurance.
- Must pass a Department of Justice/Fingerprint and Child Abuse Index clearance.
KEY RESPONSIBILITIES:
Client Care & Case Management
- Conduct comprehensive client assessments to determine medical, behavioral health, and social service needs.
- Develop, implement, and regularly update trauma-informed Individualized Care Plans based on client goals and preferences.
- Provide warm handoffs by attending healthcare, behavioral health, or community resource appointments with clients as needed to remove barriers to access and support successful engagement.
- Conduct home visits, community-based meetings, and phone outreach to support client engagement and service access.
- Advocate for client needs across healthcare, housing, legal, and social service systems.
- Maintain accurate, timely case notes and documentation in the Electronic Health Record (EHR) system.
Care Coordination & Collaboration
- Collaborate closely with managed care organizations (MCOs), healthcare providers, behavioral health specialists, and community partners.
- Coordinate healthcare appointments, transportation services, medication management support, and public benefits access.
- Participate actively in interdisciplinary case staffing meetings to review client progress and refine care strategies.
- Assist with crisis intervention, de-escalation, and emergency safety planning as needed.
Compliance & Documentation
- Ensure compliance with CalAIM ECM guidelines, Medi-Cal documentation standards, and confidentiality requirements (HIPAA).
- Meet monthly service engagement and billing requirements.
- Submit timely reports and tracking data to support program evaluation and quality improvement initiatives.
- Stay informed on state and county policy changes affecting ECM service delivery.
Client Empowerment & Health Promotion
- Educate clients on self-advocacy, healthcare system navigation, and chronic disease management.
- Support trauma-informed, strengths-based approaches that empower clients to make informed health and lifestyle decisions.
- Encourage client engagement in community wellness programs, peer support groups, and mental health services.
